【摘要】：背景:至20世紀(jì)80年代,耐甲氧西林金黃色葡萄球菌感染已經(jīng)遍布全球,目前已成為院內(nèi)感染的首要病原菌。目的:了解燒傷病房金黃色葡萄球菌的分子型別及其對(duì)臨床常用抗生素的耐藥情況,為防控耐甲氧西林金黃色葡萄球菌感染提供依據(jù)。方法:收集解放軍蘭州軍區(qū)烏魯木齊總醫(yī)院燒傷病房53株金黃色葡萄球菌,運(yùn)用PCR方法以及頭孢西丁實(shí)驗(yàn)鑒定耐甲氧西林金黃色葡萄球菌菌株,并且利用spa分型、SCCmec分型、MLST分型方法對(duì)耐甲氧西林金黃色葡萄球菌進(jìn)行分子分型,同時(shí)測(cè)定所有金黃色葡萄球菌對(duì)苯唑西林等17種藥物的敏感性,分析不同型別金黃色葡萄球菌特別是耐甲氧西林金黃色葡萄球菌的耐藥情況。結(jié)果與結(jié)論:(1)53株金黃色葡萄球菌中有41株檢測(cè)到meoA基因,另有2株頭孢西丁實(shí)驗(yàn)陽性,均定義為耐甲氧西林金黃色葡萄球菌,這些耐甲氧西林金黃色葡萄球菌可分為3種SCCmec型,4種spa型,3種ST型。(2)優(yōu)勢(shì)耐甲氧西林金黃色葡萄球菌克隆為ST239-III-t030占90.7%,該克隆均對(duì)苯唑西林等10種被檢抗生素高度耐藥。(3)結(jié)果說明,該院燒傷病房耐甲氧西林金黃色葡萄球菌分離率較高,MRSA-ST239-III-t030占據(jù)絕對(duì)優(yōu)勢(shì),具有高度耐藥性,呈現(xiàn)爆發(fā)流行特征。
[Abstract]:Background: by the 1980s, methicillin-resistant Staphylococcus aureus infection has spread all over the world and has become the leading pathogen of nosocomial infection. Objective: to investigate the molecular types of Staphylococcus aureus and its resistance to antibiotics in burn ward, and to provide evidence for the prevention and control of methicillin-resistant Staphylococcus aureus infection. Methods: 53 strains of Staphylococcus aureus were collected from burn ward of Urumqi General Hospital of Lanzhou military region of PLA. The strains resistant to methicillin-resistant Staphylococcus aureus were identified by PCR method and cefoxitin test. The molecular typing of methicillin-resistant Staphylococcus aureus was carried out by using spa typing method. The sensitivity of all Staphylococcus aureus to oxacillin and other 17 drugs was determined. To analyze the drug resistance of different types of Staphylococcus aureus, especially methicillin-resistant Staphylococcus aureus. Results and conclusion meoA gene was detected in 41 of 53 strains of Staphylococcus aureus and 2 strains were positive for cefoxitin, which were defined as methicillin-resistant Staphylococcus aureus. These methicillin-resistant Staphylococcus aureus can be divided into three SCCmec types, four spa types and three St types.) the dominant methicillin-resistant Staphylococcus aureus was cloned into 90.7% of the methicillin-resistant Staphylococcus aureus, and the clones were highly resistant to oxacillin and other 10 tested antibiotics. The isolation rate of methicillin-resistant Staphylococcus aureus in burn ward of our hospital was higher than that of MRSA-ST239-III-t030.
