急性腦梗死患者血清miR-181c水平與認(rèn)知功能受損的關(guān)系

【摘要】:目的分析與急性腦梗死患者血清微小RNA-181c(miR-181c)水平與認(rèn)知功能受損的關(guān)系。方法收集158例急性腦梗死患者,其中梗死后存在認(rèn)知功能受損67例(受損組),認(rèn)知功能正常91例(正常組)。采用實(shí)時(shí)熒光定量PCR技術(shù)檢測(cè)血清miR-181c,用多元線性回歸分析法分析認(rèn)知功能受損發(fā)生的影響因素,以受試者工作特征曲線(ROC曲線)評(píng)估血清miR-181c預(yù)測(cè)認(rèn)知功能受損的價(jià)值。結(jié)果受損組及正常組血清miR-181c相對(duì)表達(dá)量為1.57±0.19、1.32±0.15,兩組比較P0.05。多元線性回歸分析顯示,血清miR-181c、年齡、HbA_(1c)、血Hcy、相關(guān)腦功能區(qū)梗死是急性腦梗死患者發(fā)生認(rèn)知功能受損的獨(dú)立影響因素(P均0.05)。ROC曲線分析顯示,當(dāng)血清miR-181c截?cái)嘀禐?.33時(shí),其預(yù)測(cè)急性腦梗死患者發(fā)生認(rèn)知功能受損的曲線下面積為0.813(95%CI:0.746~0.880),靈敏度為91.0%,特異度為58.2%。結(jié)論急性腦梗死后認(rèn)知功能受損患者血清miR-181c水平升高,血清miR-181c水平升高是急性腦梗死后認(rèn)知功能受損的獨(dú)立危險(xiǎn)因素和預(yù)測(cè)指標(biāo)。
[Abstract]:Objective to analyze the relationship between serum minimal RNA-181cmmiR-181c and cognitive impairment in patients with acute cerebral infarction (ACI).Methods among 158 patients with acute cerebral infarction, there were 67 cases with cognitive impairment after infarction (group A) and 91 cases with normal cognitive function (n = 91).Real time fluorescence quantitative PCR was used to detect miR-181c. the influencing factors of cognitive impairment were analyzed by multivariate linear regression analysis, and the value of serum miR-181c in predicting cognitive impairment was evaluated by operating characteristic curve (ROC curve).Results the relative expression of serum miR-181c in impaired group and normal group was 1.57 鹵0.191.32 鹵0.15, compared with that in two groups (P0.05).Multivariate linear regression analysis showed that serum miR-181c, age of HbACTs 1c, blood Hcyand related cerebral functional area infarction were independent influencing factors of cognitive impairment in patients with acute cerebral infarction (ACI). The results of 0.05).ROC curve showed that when serum miR-181c truncation value was 1.33,The area under the curve of predicting cognitive impairment in patients with acute cerebral infarction was 0.81395% CI: 0.746 ~ 0.880%, sensitivity was 91.0, specificity was 58.2%.Conclusion Serum miR-181c level and serum miR-181c level in patients with cognitive impairment after acute cerebral infarction are independent risk factors and predictors of cognitive impairment after acute cerebral infarction.
