活化蛋白C對大鼠重癥急性胰腺炎相關(guān)腎損害的保護(hù)作用研究

【摘要】:[目的] 探討活化蛋白C (actived protein C APC)對重癥急性胰腺炎(SAP)相關(guān)腎損害的治療作用及機(jī)制。 [方法] 健康成年雌鼠Sprague-Dawley (SD)大鼠隨機(jī)分成四組:①假手術(shù)組(5只);②SAP組(5只);③治療對照組(5只);④治療組(8只)。假手術(shù)組:隨機(jī)取5只作為假手術(shù)組大鼠,開腹后輕輕翻動胰腺和十二指腸后關(guān)腹;SAP組、治療對照組、治療組:隨機(jī)取大鼠多只,用5%�;悄懰徕c(1ml/kg)進(jìn)行胰膽管逆行注射,誘導(dǎo)SAP模型后5小時以大鼠內(nèi)眥靜脈血測血清肌酐值篩選出SAP并發(fā)腎功能損傷(肌酐大于98μmol/1)的大鼠18只,取其中5只作為SAP組大鼠;再取5只并經(jīng)尾靜脈注射生理鹽水0.5ml作為治療對照組;最后剩余大鼠8只,將相應(yīng)大鼠治療量的APC (50μg/kg)溶解于0.5ml生理鹽水中后,注射到8只大鼠尾靜脈中作為治療組大鼠。四組動物在完成假手術(shù)或者誘導(dǎo)SAP模型后16個小時處死,收集血液,胰腺組織和腎組織標(biāo)本。血液標(biāo)本用于測定血清淀粉酶(AMY),Cr(肌酐)和BUN(血尿素氮)以及血清腫瘤壞死因子-a(TNF-a)和白介素-8(IL-8)水平(ELISA法),胰腺組織和腎組織標(biāo)本用于病理切片進(jìn)行病理學(xué)觀察并評分。 [結(jié)果] 1治療組血清AMY水平明顯低于治療對照組(P0.01);SAP組該指標(biāo)明顯高于假手術(shù)組(P0.01)。 2治療組胰腺組織損傷病理學(xué)評分、腎組織損傷病理學(xué)評分明顯低于治療對照組(P0.01);SAP組該兩種指標(biāo)均明顯高于假手術(shù)組(P0.01)。 3治療組TNF-α、IL-8、Cr、BUN水平均低于治療對照組(P0.05);SAP組該四種指標(biāo)均明顯高于假手術(shù)組(P0.01)。 [結(jié)論] 活化蛋白C可降低IL-8與TNF-α水平及炎癥反應(yīng),對重癥急性胰腺炎相關(guān)腎損害具有保護(hù)作用。
[Abstract]:[purpose]To investigate the therapeutic effect and mechanism of activated protein C APC on renal damage associated with severe acute pancreatitis (SAP).[methods]Healthy adult female rats (Sprague-Dawley) were randomly divided into four groups: one sham-operated group (n = 5), a sham-operated group (n = 5), a control group (n = 5), a control group (n = 5) and a control group (n = 5).At 5 hours after induction of SAP model, 18 SAP rats complicated with renal function damage (creatinine > 98 渭 mol / 1) were selected from the serum creatinine values of the inner canthus vein of rats. 5 of them were taken as SAP group rats.Five rats were injected with saline 0.5ml via caudal vein as the control group, and the remaining 8 rats were treated with the corresponding dosage of APC 50 渭 g / kg, then injected into the tail vein of 8 rats as the treatment group.The animals in the four groups were killed 16 hours after sham-operation or SAP model. Blood, pancreas and kidney were collected.Blood samples were used to measure serum amylase (Cr) and bun (blood urea nitrogen), serum tumor necrosis factor (TNF- 偽) and interleukin-8 (IL-8) levels were determined by Elisa. Pancreatic and renal specimens were used for pathological observation and grading.[results]1the serum AMY level in the treatment group was significantly lower than that in the control group (P 0.01) and was significantly higher than that in the sham operation group (P 0.01).2the pathological scores of pancreatic injury and renal injury in the treatment group were significantly lower than those in the control group (P 0.01) and SAP group (P 0.01), which were significantly higher than those in the sham operation group (P 0.01).(3) the levels of TNF- 偽 IL-8Cr bun in the treatment group were lower than those in the control group (P 0.05) and SAP group were significantly higher than those in the sham operation group (P 0.01).[conclusion]Activated protein C can reduce the levels of IL-8 and TNF- 偽 and the inflammatory response, and has protective effect on renal damage associated with severe acute pancreatitis.
