顱腦損傷患者入院后不同時(shí)間段血清中Tau蛋白濃度的診斷價(jià)值

【摘要】:目的通過(guò)檢測(cè)顱腦損傷(TBI)患者入院后不同時(shí)間段血清中的Tau蛋白濃度,對(duì)照比較TBI患者標(biāo)準(zhǔn)化治療后的預(yù)后情況。方法篩選無(wú)TBI及相關(guān)顱腦疾病的體檢者為對(duì)照組,檢測(cè)對(duì)照組血清中Tau蛋白的濃度。收集臨床確診的TBI患者76例,于傷后的6 h及1、3、7 d分別采集患者血清標(biāo)本。通過(guò)ELISA法分別檢測(cè)對(duì)應(yīng)時(shí)間段內(nèi)的血清Tau蛋白濃度。于患者入院標(biāo)準(zhǔn)化治療1個(gè)月后進(jìn)行隨訪或復(fù)診計(jì)算患者預(yù)后分?jǐn)?shù)(通過(guò)Barthel指數(shù)評(píng)分計(jì)算)。應(yīng)用SPSS統(tǒng)計(jì)軟件對(duì)患者血清中Tau濃度、GCS評(píng)分、Barthel指數(shù)評(píng)分進(jìn)行相關(guān)性分析。結(jié)果各TBI組血清Tau蛋白濃度與對(duì)照組相比較,差異有統(tǒng)計(jì)學(xué)意義(P0.05);重型組與輕中型組相比較,其血清Tau蛋白濃度差異有統(tǒng)計(jì)學(xué)意義(P0.05)。重型組患者的預(yù)后明顯差于輕中型組。預(yù)后不同的各組血清中Tau蛋白濃度相比較,預(yù)后差組、預(yù)后中組、預(yù)后良組與對(duì)照組差異有統(tǒng)計(jì)學(xué)意義(P0.05);預(yù)后差組與預(yù)后中組、預(yù)后良組差異有統(tǒng)計(jì)學(xué)意義(P0.05);預(yù)后中組與預(yù)后良組差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論在發(fā)生TBI后,患者血清中Tau蛋白含量升高,升高的程度與TBI的嚴(yán)重程度呈正相關(guān)。TBI的程度與患者預(yù)后有關(guān),損傷越重,患者預(yù)后越差。預(yù)后差患者與患者血清中Tau蛋白含量相關(guān),Tau蛋白含量極高者預(yù)后差。
[Abstract]:Objective to detect the concentration of Tau protein in serum of patients with craniocerebral injury (TBI) at different time after admission, and to compare the prognosis of patients with TBI after standardized treatment.Methods the control group was selected as the control group without TBI and related craniocerebral diseases. The concentration of Tau protein in serum of the control group was detected.The serum samples of 76 patients with TBI were collected at 6 hours and 3 days after injury.The concentration of serum Tau protein was detected by ELISA method.The prognostic scores (calculated by Barthel index score) were calculated 1 month after admission to standardized therapy.The correlation between Tau and Barthel index was analyzed by SPSS software.Results compared with the control group, the concentration of serum Tau protein in the TBI group was significantly different from that in the control group (P 0.05), and the serum Tau protein concentration in the severe group was significantly higher than that in the light group (P 0.05).The prognosis of severe group was significantly worse than that of light and medium group.There were significant differences in serum Tau protein concentration between groups with different prognosis, poor prognosis group, prognosis group, good prognosis group and control group (P 0.05), poor prognosis group and prognosis group.There was significant difference between the good group and the good group (P 0.05), but there was no significant difference between the group with good prognosis and the group with good prognosis (P 0.05).Conclusion after the onset of TBI, the serum Tau protein level increases, and the degree of elevation is positively correlated with the severity of TBI. The degree of TBI is related to the prognosis of the patients. The more serious the injury, the worse the prognosis of the patients.The prognosis of patients with poor prognosis and patients with extremely high Tau protein content was poor.
