【摘要】：目的:探討淺低溫低流量體外循環(huán)心臟不停跳下肺動脈血栓內(nèi)膜剝脫術(shù)治療效果,以提高慢性肺動脈栓塞及大型急性肺動脈栓塞外科治療的安全性。方法:回顧性分析2012年1月至2015年12月廣西醫(yī)科大學(xué)第一附屬醫(yī)院16例肺栓塞患者,在淺低溫低流量體外循環(huán)下行肺動脈血栓內(nèi)膜剝脫術(shù)。結(jié)果:其中肺動脈收縮壓和肺血管阻力較前降低,動脈血氧飽和度、動脈血氧分壓、心排量、心臟指數(shù)較前增高,心功能較前顯著改善。結(jié)論:對于慢性肺栓塞及內(nèi)科治療無效的急性大面積肺栓塞患者采用外科手術(shù)治療,是目前安全、有效的治療方法。
[Abstract]:Objective: to investigate the effect of endarterectomy of pulmonary artery thrombus under mild hypothermia and low flow cardiopulmonary bypass (CPB) in order to improve the safety of surgical treatment of chronic pulmonary embolism and large acute pulmonary embolism.Methods: from January 2012 to December 2015, 16 patients with pulmonary embolism in the first affiliated Hospital of Guangxi Medical University were analyzed retrospectively. Pulmonary artery thrombus endarterectomy was performed under mild hypothermia and low flow cardiopulmonary bypass.Results: pulmonary artery systolic pressure and pulmonary vascular resistance were decreased, arterial oxygen saturation, arterial oxygen partial pressure, cardiac output, cardiac index and cardiac function were significantly improved.Conclusion: surgical treatment is a safe and effective treatment for patients with chronic pulmonary embolism and acute large area pulmonary embolism.
