【摘要】：本研究的目的是評估急性心肌梗死患者院前延誤的原因,包括社會經(jīng)濟(jì)學(xué)和個人因素等。選取327例急性ST段抬高型心肌梗死(STEMI)患者,詳細(xì)記錄患者人口學(xué)資料,受教育水平,婚姻狀況,何種方式就診于醫(yī)院及院前延誤時間。35.7%的患者在癥狀發(fā)作的2小時內(nèi)到達(dá)醫(yī)院,7.9%患者在癥狀發(fā)作24小時后到達(dá)醫(yī)院。經(jīng)多因素回歸分析,已婚,高學(xué)歷,既往有心肌梗死病史,將自己的癥狀歸結(jié)為心臟原因,具有的疼痛癥狀和非疼痛癥狀數(shù)量多,發(fā)病時不在家,發(fā)病時有陪同者,發(fā)病后呼叫EMS是院前延誤時間短的相關(guān)因素(P0.05)。增加患者對心血管癥狀和危險因素的認(rèn)識,教育患者一旦出現(xiàn)癥狀,立即呼叫急救系統(tǒng)可能縮短院前延誤時間。
[Abstract]:The purpose of this study was to evaluate the causes of pre-hospital delay in patients with acute myocardial infarction, including socioeconomic and personal factors. 327 patients with acute ST-segment elevation myocardial infarction (STEMI) were selected to record the demographic data and educational level of the patients in detail. Marital status, how to see a doctor in hospital and the delay before hospital. 35.7% of the patients arrived in hospital within 2 hours after symptom attack. 7.9% of the patients arrived in hospital 24 hours after symptom attack. Multivariate regression analysis showed that they were married and had higher education. Having a history of myocardial infarction in the past, I attributed my symptoms to cardiac causes, with a large number of pain and non-pain symptoms, not at home at the time of onset, and accompanied at the time of onset. Calling EMS after onset is a relative factor of short delay time before hospital (P 0.05). To increase the understanding of cardiovascular symptoms and risk factors, to educate patients to call the first aid system immediately may shorten the time of delay before hospital.
