【摘要】：目的:探討膿毒癥心肌損傷患者心型脂肪酸結(jié)合蛋白(H-FABP)和N-末端腦鈉肽前體(NTpro BNP)的變化及臨床意義。方法 :118例膿毒癥患者按預(yù)后分為存活組68例和死亡組50例;體檢者50例為對(duì)照組。膿毒癥患者入院1 h(發(fā)病3 h內(nèi))、6 h時(shí),對(duì)照組體檢時(shí)檢測(cè)心肌酶、H-FABP及NT-pro BNP水平、APACHEⅡ評(píng)分及28 d病死率。結(jié)果:入院1 h,膿毒癥患者H-FABP、NT-pro BNP和APACHEⅡ評(píng)分高于對(duì)照組(P0.05),6 h時(shí)c Tn I、H-FABP、NT-pro BNP和APACHEⅡ評(píng)分高于1 h(P0.05);死亡組在1 h和6 h時(shí)H-FABP、NT-pro BNP和APACHEⅡ評(píng)分高于存活組(P0.05);6 h時(shí)死亡組c Tn I高于存活組(P0.05)�；颊�6 h時(shí)H-FABP和NT-pro BNP與c Tn I呈正相關(guān)(均P0.05)。6 h時(shí)H-FABP對(duì)28 d病死率的預(yù)測(cè)能力優(yōu)于NT-pro BNP及APACHEⅡ評(píng)分(P0.05)。結(jié)論:H-FABP和NT-pro BNP對(duì)膿毒癥患者心肌損傷的早期診斷和預(yù)后評(píng)估有重要意義。
[Abstract]:Objective: to investigate the changes and clinical significance of cardiac fatty acid binding protein (H-FABP) and N-terminal brain natriuretic peptide precursor (NTpro BNPP) in septic myocardial injury patients. Methods: according to the prognosis, 118 septic patients were divided into survival group (n = 68) and death group (n = 50). 50 cases of physical examination served as control group. Sepsis patients were admitted to hospital for 1 hour (within 3 hours after onset of the disease and 6 hours after onset).
