急性腦梗死患者血清促動脈硬化指數與頸動脈內膜中層厚度及氧化型低密度脂蛋白水平的研究

【摘要】:目的探討急性腦梗死(ACI)患者血清氧化型低密度脂蛋白(ox-LDL)、促動脈硬化指數(AIP)水平與頸動脈粥樣硬化的關系。方法連續(xù)性選取ACI患者120例作為研究對象,選擇同期健康體檢者60例作為對照組。分別檢測兩組的ox-LDL水平及血漿脂質代謝水平,后者包括甘油三酯(TG)、低密度脂蛋白(LDL-C)、高密度脂蛋白(HDL-C),并計算血漿促動脈硬化指數(AIP)。對ACI組患者進行頸部血管彩超檢查,根據檢查結果將其分為頸動脈內膜中層厚度(IMT)正常組(11例)、IMT增厚組(25例)和頸動脈粥樣斑塊形成(CAS)組(84例),比較各組ox-LDL和AIP水平。結果與對照組相比,ACI組斑塊檢出率、易損斑塊檢出率明顯增高(P0.01);ACI組血清ox-LDL與AIP明顯升高(P0.01)。在ACI患者中,CAS組血清ox-LDL及AIP較IMT增厚組明顯增高(P0.01);IMT增厚組血清ox-LDL及AIP高于IMT正常組(P0.01,P0.05)。Pearson檢驗結果顯示,ox-LDL水平與IMT水平呈正相關(r=0.720,P0.01);AIP值與IMT水平呈正相關(r=0.717,P0.01);血清AIP與ox-LDL水平之間有相關性(r=0.655,P0.01)。結論 ox-LDL和AIP與頸動脈粥樣硬化形成、發(fā)展及急性腦梗死發(fā)生有密切關系。兩者聯合測定能夠更全面評估缺血性腦卒中發(fā)生的風險。
[Abstract]:Objective to investigate the relationship between serum oxidized low density lipoprotein (ox-LDL) and atherosclerosis index (AIP) in patients with acute cerebral infarction (ACI). 60 healthy persons were selected as control group. The levels of ox-LDL and plasma lipid metabolism were measured in the two groups. The latter included triglyceride, low density lipoprotein (LDL-CU), high density lipoprotein (HDL-C), and plasma arteriosclerotic index (AIP). The patients in ACI group were examined by color Doppler ultrasonography. According to the examination results, it was divided into 11 cases of carotid intima-media thickness (IMT) normal group (25 cases) and 84 cases of carotid atherosclerotic plaque formation (Cass) group. The levels of ox-LDL and AIP in each group were compared. Results compared with the control group, the detection rate of plaque in ACI group was higher than that in control group. The detection rate of vulnerable plaques was significantly higher in P0.01ACI group than in P0.01ACI group. The serum ox-LDL and AIP levels in ACI group were significantly higher than those in IMT thickening group. The results of ox-LDL and AIP were higher in ACI group than those in normal IMT group (P 0.01 ox-LDL P 0.05 .Pearson test). The level of IMT was positively correlated with the level of IMT, and the level of serum AIP was correlated with the level of ox-LDL. Conclusion ox-LDL and AIP are associated with the formation of carotid atherosclerosis. There is a close relationship between development and acute cerebral infarction.
