【摘要】：目的通過(guò)超聲檢測(cè)探討急性腦梗死(ACI)患者頸動(dòng)脈及椎動(dòng)脈結(jié)構(gòu)和血流動(dòng)力學(xué)特征。方法對(duì)298例ACI患者及101例健康對(duì)照者行頸動(dòng)脈及椎動(dòng)脈超聲檢測(cè),分析兩組間血管內(nèi)徑、內(nèi)膜中膜厚度(IMT)、血管阻力指數(shù)(RI)及血流速度等方面的差異。根據(jù)NIHSS評(píng)分,把ACI組分為2個(gè)亞組:進(jìn)展性ACI組和非進(jìn)展性ACI組,比較2個(gè)亞組間上述超聲參數(shù)的差異。結(jié)果 (1)ACI組頸總動(dòng)脈內(nèi)徑、頸內(nèi)動(dòng)脈內(nèi)徑、頸總動(dòng)脈RI、左側(cè)頸內(nèi)動(dòng)脈RI均大于健康對(duì)照組(P0.05);ACI組頸總動(dòng)脈舒張末期血流速度(Vd)、頸內(nèi)動(dòng)脈收縮期峰值血流速度(Vs)、頸內(nèi)動(dòng)脈Vd、右側(cè)椎動(dòng)脈Vs及Vd均低于健康對(duì)照組(P0.05)。(2)進(jìn)展性ACI組左側(cè)頸總動(dòng)脈Vs、左側(cè)頸總動(dòng)脈RI、頸內(nèi)動(dòng)脈IMT高于非進(jìn)展性ACI組(P0.05);進(jìn)展性ACI組右側(cè)椎動(dòng)脈Vs低于非進(jìn)展性ACI組(P0.05)。(3)Pearson相關(guān)性分析表明,年齡與各血管超聲參數(shù)相關(guān)。結(jié)論頸動(dòng)脈粥樣硬化血管結(jié)構(gòu)失代償是ACI的特征之一,頸內(nèi)動(dòng)脈IMT增厚可能是ACI進(jìn)展加重的危險(xiǎn)信號(hào)。
[Abstract]:Objective to investigate the structure and hemodynamic characteristics of carotid and vertebral arteries in patients with acute cerebral infarction (ACI) by ultrasonography. According to the NIHSS score, the ACI group was divided into two subgroups: progressive ACI group and non-progressive ACI group. Results the internal diameter of common carotid artery and internal carotid artery in ACI group were compared. The RI of the common carotid artery and the RI of the left internal carotid artery were higher than those of the healthy control group (P 0.05). The peak systolic blood flow velocity of the common carotid artery and the systolic blood flow velocity of the internal carotid artery, the right vertebral artery Vs and VD were lower than those of the healthy control group. The correlation analysis showed that the left common carotid artery vs, left common carotid artery RI, internal carotid artery IMT in the progressive ACI group were higher than those in the non-progressive ACI group, and the Vs of the right vertebral artery in the progressive ACI group was lower than that in the non-progressive ACI group. Conclusion decompensation of carotid atherosclerotic vascular structure is one of the characteristics of ACI. The thickening of internal carotid IMT may be a dangerous signal for the progression of ACI.
