烏司他丁對(duì)膿毒癥急性腎損傷患者心鈉肽、尿腎損傷分子-1以及血清半胱氨酸蛋白酶抑制劑的影響

【摘要】:目的探討烏司他丁對(duì)膿毒癥急性腎損傷(AKI)患者心鈉肽(ANP)、尿腎損傷分子-1(KIM-1)以及血清半胱氨酸蛋白酶抑制劑(CYS)的影響。方法將患者隨機(jī)分為對(duì)照組和觀察組,對(duì)照組患者在入院后及時(shí)進(jìn)行抗感染、重癥監(jiān)護(hù)、呼吸輔助及營(yíng)養(yǎng)支持等常規(guī)治療;觀察組患者采用常規(guī)治療基礎(chǔ)上聯(lián)合烏司他丁治療,兩組患者均進(jìn)行1個(gè)療程的治療;檢測(cè)患者尿液樣本中ANP和KIM-1含量及血液樣本中CYS含量,并對(duì)患者治療前后急性生理與慢性健康評(píng)分進(jìn)行評(píng)估分析。結(jié)果對(duì)照組患者尿液樣本中ANP水平逐漸增加,而觀察組則于治療第3天時(shí)達(dá)到峰值之后逐漸下降,且在治療3 d后兩組間差異具有統(tǒng)計(jì)學(xué)意義(t=2.91及8.59,P0.05);對(duì)照組患者尿液樣本中KIM-1及血液樣本中CYS水平逐漸遞增,而觀察組則于治療第5天時(shí)達(dá)到峰值之后逐漸下降,在治療第7天時(shí)兩組間差異具有統(tǒng)計(jì)學(xué)意義(t=11.23及35.89,P0.05);此外,兩組組內(nèi)及組間不同時(shí)間點(diǎn)比較,其尿液樣本中ANP、KIM-1及血液樣本中CYC水平均存在顯著性差異(F=7.32、10.16、5.89及19.74、21.05、11.63,P0.05);治療前后兩組患者APACHEⅡ存在顯著性差異(t=16.04及35.49,P0.05),且治療后兩組間亦存在顯著性差異(t=18.80,P0.05),但治療前兩組間差異無(wú)統(tǒng)計(jì)學(xué)意義(t=0.71,P0.05)。結(jié)論烏司他丁能夠顯著降低膿毒癥急性腎損傷患者體內(nèi)ANP、KIM-1以及CYS水平,對(duì)腎損傷具有一定的保護(hù)作用。
[Abstract]:Objective to investigate the effect of ulinastatin on atrial natriuretic peptide (ANPP) in patients with acute renal injury (AKI) of sepsis. Methods the patients were randomly divided into two groups: control group and observation group. The patients in the control group were treated with routine therapy such as anti-infection, intensive care, respiratory assistance and nutritional support in time after admission. The patients in the observation group were treated with routine therapy combined with ulinastatin, and both groups were treated with one course of treatment. The levels of ANP and KIM-1 in urine and CYS in blood samples were measured. The scores of acute physiology and chronic health before and after treatment were evaluated and analyzed. Results the level of ANP in urine samples of patients in control group increased gradually. On the third day after treatment, the peak value of the observation group was decreased gradually, and the difference between the two groups was statistically significant after 3 days of treatment (2.91 and 8.59 P0.05). In the control group, the levels of KIM-1 in urine and CYS in blood samples increased gradually, while the levels of CYS in the observation group decreased gradually after reaching the peak on the 5th day after treatment. On the 7th day of treatment, the difference between the two groups was statistically significant (11.23 and 35.89) (P 0.05).
