產(chǎn)OXA-23鮑曼不動(dòng)桿菌可移動(dòng)遺傳元件研究

【摘要】:目的了解產(chǎn)OXA-23鮑曼不動(dòng)桿菌(Acinetobacter baumannii,Ab)中整合子、轉(zhuǎn)座子、插入序列及接合性質(zhì)粒等可移動(dòng)遺傳元件分布狀況及其與耐藥性的關(guān)系。方法收集中南大學(xué)湘雅醫(yī)院2009年1~6月產(chǎn)OXA-23鮑曼不動(dòng)桿菌71株;根據(jù)CLSI2009年標(biāo)準(zhǔn)采用紙片擴(kuò)散法進(jìn)行藥物敏感性試驗(yàn);聚合酶鏈?zhǔn)椒磻?yīng)(PCR)檢測(cè)3種整合子遺傳標(biāo)記(intI 1、intI 2和intI 3)、4種轉(zhuǎn)座子遺傳標(biāo)記(merA、tnpU、tnp513和tnpA)、2種插入序列遺傳標(biāo)記(ISAba1、IS26)及2種接合性質(zhì)粒遺傳標(biāo)記(traA、trbC),共11種可移動(dòng)遺傳元件;結(jié)合藥敏結(jié)果分析可移動(dòng)遺傳元件與耐藥性的關(guān)系。結(jié)果 intI 1、merA、tnpU、tnp513、ISAba1和IS26檢出陽(yáng)性率分別為:85.9%、7.0%、91.5%、83.1%、100.0%和100.0%。其余5種可移動(dòng)遺傳元件未檢出;71株鮑曼不動(dòng)桿菌每株至少檢出2種可移動(dòng)遺傳元件,最多檢出6種可移動(dòng)遺傳元件,其中大部分受試菌檢出5種可移動(dòng)遺傳元件;在國(guó)內(nèi)首次從鮑曼不動(dòng)桿菌中檢出merA基因,且首次在國(guó)內(nèi)同時(shí)從1株鮑曼不動(dòng)桿菌中檢出intI1、merA、tnpU、tnp513、ISAba1和IS26共6種可移動(dòng)遺傳元件;對(duì)n5和n≥5兩組耐10種及以上藥物菌株比例差異進(jìn)行χ2檢驗(yàn)發(fā)現(xiàn),差異具有顯著性(P0.05),提示可移動(dòng)遺傳元件檢出數(shù)越高,耐多藥菌株數(shù)越多,耐藥率越高。結(jié)論產(chǎn)OXA-23鮑曼不動(dòng)桿菌易檢出各類可移動(dòng)遺傳元件,與醫(yī)院鮑曼不動(dòng)桿菌耐藥機(jī)制相關(guān)的可移動(dòng)遺傳元件主要有intI 1、merA、tnpU、tnp513、ISAba1和IS26。
[Abstract]:Aim to investigate the distribution of moveable genetic elements in Acinetobacter baumannii (Acinetobacter baumannii,Ab) producing OXA-23, such as integron, transposon, insertion sequence and conjugative plasmid, and their relationship with drug resistance of Acinetobacter baumannii (Acinetobacter baumannii,Ab). Methods 71 strains of Acinetobacter baumannii produced in Xiangya Hospital of Central South University from January to June 2009 were collected. Polymerase chain reaction (PCR) was used to detect three integrated genetic markers (intI 1, inti 2 and intI 3) and four transposon genetic markers (merA,tnpU,tnp513 and tnpA), insertion sequence genetic markers) (ISAba1,).
