活體豬椎體多極射頻消融的熱場分布

【摘要】:目的通過對活體豬椎體進(jìn)行射頻消融(RFA),觀察熱場分布,探討RFA時(shí)電極針在椎體中的位置與脊髓的安全性關(guān)系以及椎體周圍有無軟組織損傷。方法取2頭小型香豬,1頭消融2個(gè)椎體后即刻取標(biāo)本制作病理切片;另1頭消融10個(gè)椎體,每周消融2個(gè)椎體,完成后第6周取標(biāo)本制作病理切片。每個(gè)椎體消融時(shí)按預(yù)先設(shè)計(jì)的測溫點(diǎn)測"0"時(shí)刻、消融5、10、15、20 min各測溫點(diǎn)的溫度值。結(jié)果各測溫點(diǎn)"0"時(shí)刻溫度值差異無統(tǒng)計(jì)學(xué)意義(P0.05)。當(dāng)消融5 min后電極針尖溫度達(dá)到90℃以上時(shí),各測溫點(diǎn)的溫度值差異有統(tǒng)計(jì)學(xué)意義(P0.05)。電極針尖及椎體骨質(zhì)內(nèi)深1 cm位置的溫度在消融5 min后所測得的值明顯高于椎體后壁及椎體的骨皮質(zhì)表面,但各測溫點(diǎn)的溫度均高于"0"時(shí)刻各測溫點(diǎn)的溫度值。椎體骨質(zhì)內(nèi)深1 cm位置的溫度在消融5 min后4個(gè)記錄時(shí)刻比較差異有統(tǒng)計(jì)學(xué)意義(P0.05),溫度總體上呈漸升過程。椎體后壁及椎體骨皮質(zhì)表面的溫度在消融5 min后4個(gè)記錄時(shí)刻的溫度比較差異無統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論在對椎體行RFA時(shí),椎管內(nèi)流動(dòng)的腦脊液及椎體骨質(zhì)內(nèi)流動(dòng)的血液會(huì)帶走部分熱量;骨皮質(zhì)具有一定的隔熱作用,對椎體后壁保持完整的椎體轉(zhuǎn)移性腫瘤RFA治療安全、可靠。
[Abstract]:Objective to observe the distribution of thermal field by radiofrequency ablation (RFA) in porcine vertebrae and to investigate the relationship between the position of electrode needle in the vertebra and the safety of spinal cord and the soft tissue injury around the vertebral body. Methods pathological sections were made immediately after ablation of 2 vertebrae in 2 miniature pigs, 10 vertebrae in the other one, 2 vertebrae per week, and 6 weeks after ablation. The "0" time of each vertebral body was measured according to the pre-designed temperature measurement point, and the temperature values of each temperature measuring point were measured at 510 ~ 1520 min.
