【摘要】：研究背景隨著社會的進步、發(fā)展和不斷變革,人們在享受著這一切帶來的高效、便捷生活方式的同時,也承受著越來越大的壓力。尤其是生活在城市的居民,要面臨更多的工作、學習、擇業(yè)等壓力,面對更激烈的競爭,這時,人們往往更容易處于各種急、慢性應激狀態(tài)。適度的應激可以提高機體的適應能力,使人保持上進心,但是過度的應激持續(xù)時間長,就會成為適應性的負擔,久而久之就會導致機體焦慮。研究表明,長期處于過度應激狀態(tài)易于導致焦慮,目前80%的疾病均與過度應激有密切的關(guān)系。長時間的持續(xù)應激,神經(jīng)內(nèi)分泌系統(tǒng)紊亂,引起相關(guān)炎性細胞因子的分泌、合成和轉(zhuǎn)輸異常。熱休克蛋白70(HSP70)作為重要的保護蛋白,參與調(diào)控炎性細胞因子的表達。臨床上的抗焦慮藥物存在一定的副作用,研究者逐步把目光投向了中醫(yī)學領(lǐng)域。研究目的本課題主要探討逍遙散對慢性束縛應激焦慮模型大鼠胃黏膜HSP70及相關(guān)炎性細胞因子的調(diào)節(jié)作用,以亟探索逍遙散保護因情志不適而導致的胃黏膜損傷的作用機制,為臨床應用逍遙散治療因情志不適所導致的胃黏膜損傷提供實驗依據(jù)。研究方法將48只健康的雄性SD大鼠(體重為220-250g)隨機分為正常組、模型組、逍遙散組、咯利普蘭組,每組分別12只。分組后的大鼠適應性喂養(yǎng)10d,實驗組均以慢性束縛應激的方法制備焦慮大鼠模型:每天3h,連續(xù)14d。逍遙散組灌胃逍遙散懸浮液,而正常組和模型組分別灌胃去離子水,咯利普蘭組注射相應藥物。分別在第0、7、14d對大鼠進行體重測量、高架十字迷宮行為學測試;在實驗的第0、14d做新環(huán)境進食抑制實驗。每天定時觀察大鼠的精神狀態(tài)、身體姿勢、皮毛色澤、活動度、對束縛的反應程度、眼黏膜色澤、耳廓色澤以及糞便等一般情況。通過免疫組化法檢測胃黏膜HSP70的表達量。通過ELISA實驗檢測血清中IL-6、IL-10等相關(guān)炎性細胞因子的含量。結(jié)果1.一般狀態(tài),兩周造模結(jié)束時,正常組、逍遙散組和咯利普蘭組大鼠明顯比模型組大鼠動作靈活,叫聲柔和,嘶叫、反抗、掙扎的次數(shù)也明顯較少,精神狀態(tài)也尚可,毛發(fā)也比模型組整齊,糞便干濕適中呈球狀;而模型組大鼠萎靡不振,毛發(fā)凌亂,色澤不明,蜷伏在角落里呈警惕狀態(tài),束縛時的反抗、嘶叫、掙扎也比一周前稍有減弱。2.體重變化,造模前1d及造模后第7d,各組大鼠體重未見明顯差異,到第14d,與模型組相比,其余三組大鼠平均體重高于模型組,有統(tǒng)計學差異(P0.05),與正常組相比,逍遙散組及咯利普蘭組大鼠體重無顯著性差異。3.新環(huán)境進食抑制實驗,在實驗開始時,各組無統(tǒng)計學差異,而造模后的第14d,模型組時間明顯多于逍遙散組。4.高架十字迷宮實驗,兩周造模實驗結(jié)束后,與正常組相比,模型組大鼠封閉臂進入次數(shù)減少,停留時間延長;與正常組相比,其余三組雙側(cè)開放臂進入次數(shù)減少且停留時間減少,有統(tǒng)計學意義;與模型組相比,逍遙散組大鼠封閉臂進入次數(shù)增多且停留時間短,但均無統(tǒng)計學差異。5.免疫組化實驗,模型組大鼠胃黏膜細胞表達的HSP70明顯低于正常組,具有統(tǒng)計學意義(P0.05),與模型組大鼠相比,逍遙散組大鼠陽性表達量明顯升高,且具有統(tǒng)計學意義(P0.01);逍遙散組和咯利普蘭組間無統(tǒng)計學差異,逍遙散組表達量略高于咯利普蘭組。6.ELISA實驗,模型組大鼠血清中的IL-1β、IL-6、IL-10和TNF-α含量明顯上升,高于其它三組,逍遙散組和咯利普蘭組間無統(tǒng)計學意義。結(jié)論14d慢性束縛應激可以導致大鼠焦慮,逍遙散可通過促進胃黏膜產(chǎn)生大量HSP70的方式來抑制相關(guān)炎性細胞因子的釋放,進而起到保護胃黏膜的作用,為臨床應用逍遙散治療因情志不適所導致的胃黏膜損傷提供實驗依據(jù)。
