2015年河南省豫東地區(qū)人體腸道寄生蟲感染現(xiàn)狀調(diào)查

【摘要】:目的了解河南省豫東地區(qū)腸道寄生蟲感染的流行現(xiàn)狀,為該地區(qū)腸道寄生蟲感染的防治提供科學依據(jù)。方法 2014年11月-2015年6月按照經(jīng)濟條件和地理方位進行分層整群抽樣,隨機選擇豫東地區(qū)的臨潁縣、濮陽縣、臺前縣、太康縣、西平縣、息縣、項城市、新鄭市、虞城縣等9個縣(市)26個調(diào)查點,采集人群糞樣,采用改良加藤厚涂片法檢查腸道蠕蟲感染情況,鉤蚴試管濾紙培養(yǎng)法鑒別鉤蟲(Ancylostoma sp.)蟲種,3~6歲兒童采用透明膠紙肛拭法檢查蟯蟲(Enterobius vermicularis)感染情況,碘液染色法檢查腸道原蟲感染情況。收集各調(diào)查點的主要飲用水源、農(nóng)民年人均純收入等基本信息。將不同地區(qū)感染率進行標準化后進行比較。不同蟲種、性別、年齡和職業(yè)以及其他相關(guān)因素的感染率間差異比較采用卡方檢驗。結(jié)果共收集糞樣6 696份,檢出腸道蠕蟲6種,腸道原蟲4種。腸道寄生蟲總感染率為1.57%(105/6 696),其中腸道蠕蟲和原蟲感染率分別為0.91%(61/6 696)和0.66%(44/6 696)。感染率較高的3種腸道寄生蟲分別為蟯蟲(0.36%,24/6 696)、人芽囊原蟲(Blastocystis hominis)(0.30%,20/6 696)和蛔蟲(Ascaris lumbricoides)(0.19%,13/6 696)。3~6歲兒童蟯蟲感染率為2.23%(15/674),其中肛拭蟯蟲感染率為1.93%(13/674)。7份鉤蚴經(jīng)鑒定均為十二指腸鉤蟲(Ancylostoma duodenale)。腸道寄生蟲感染率較高的3個縣(市)分別是項城市(4.24%)、新鄭市(3.51%)和太康縣(3.34%)。男性和女性腸道寄生蟲感染率分別為1.71%(55/3 214)和1.44%(50/3 482),兩者差異無統(tǒng)計學意義(χ~2=0.82,P0.05)。不同文化程度人群中,腸道寄生蟲感染率最高的為高中/中專組2.11%(10/473),與其他組比校,差異無統(tǒng)計學意義(χ~2=2.41,P0.05)。不同年齡組人群中,15~29歲組人群的腸道寄生蟲感染率最高,為2.38%(21/884),與其他年齡組比較,差異有統(tǒng)計學意義(χ~2=9.39,P0.01)。不同職業(yè)人群中,學齡前和學生人群的腸道寄生蟲感染率較高,為2.5%(24/720)和2.09%(24/1 148),與其他職業(yè)人群比較,差異有統(tǒng)計學意義(χ~2=9.93,P0.05)。飲用水為井水的調(diào)查點人群腸道寄生蟲感染率為1.21%(31/2 562),高于自來水飲用人群的0.65%(27/4 134)(χ~2=5.71,P0.05)。調(diào)查點農(nóng)民年人均純收入為1 200~11 600元,中位數(shù)為5 604.5元,腸道寄生蟲感染率隨農(nóng)民年人均純收入的增加而降低(χ~2=11.08,P0.01)。結(jié)論河南省豫東地區(qū)人體重點寄生蟲感染情況較前兩次調(diào)查有顯著下降,學齡前兒童、學生人群是今后監(jiān)測的重點人群。
