T細胞酶聯(lián)免疫斑點法診斷結(jié)核性腦膜炎的Meta分析

【摘要】：目的探討腦脊液和外周血T細胞酶聯(lián)免疫斑點法(T-SPOT.TB)檢測在結(jié)核性腦膜炎中的診斷價值。方法檢索1990年1月至2013年10月PubMed、EMBASE等數(shù)據(jù)庫,收集以T-SPOT.TB法診斷結(jié)核性腦膜炎的文獻。由兩位研究者獨立篩選文獻,提取資料,質(zhì)量評價,以Meta-Disc 1.4軟件進行Meta分析。結(jié)果共納入8篇文獻,425個病例,結(jié)核性腦膜炎232例。外周血組合并靈敏度為80%(95%CI:0.74~0.85),合并特異度為74%(95%CI:0.67~0.80),SROC曲線下面積為0.858 7;診斷比值比為15.50。腦脊液組合并靈敏度為76%(95%CI:0.70~0.82),合并特異度為83%(95%CI:0.77~0.88),SROC曲線下面積為0.892 7;合并診斷比值比為22.62。結(jié)論采用T-SPOT.TB法有助于提高結(jié)核性腦膜炎的診斷率。腦脊液T-SPOT.TB診斷準(zhǔn)確性可能高于外周血組。
[Abstract]:Objective to evaluate the diagnostic value of cerebrospinal fluid (cerebrospinal fluid) and peripheral blood T cell enzyme linked immunosorbent assay (T-SPOT.TB) in tuberculosis meningitis. Methods from January 1990 to October 2013, PubMed,EMBASE and other databases were searched, and the literature on the diagnosis of tuberculosis meningitis by T-SPOT.TB method was collected. The literature was screened independently by the two researchers, the data were extracted and the quality was evaluated. Meta analysis was carried out with Meta-Disc 1.4 software. Results A total of 8 articles, 425 cases and 232 cases of tuberculosis meningitis were included. In the peripheral blood group, the sensitivity was 80% (95%CI:0.74~0.85), the combined specificity was 74% (the area under the 95%CI:0.67~0.80), SROC curve was 0.858, and the diagnostic ratio was 15.50). The sensitivity of cerebrospinal fluid combination was 76% (95%CI:0.70~0.82), the combined specificity was 83% (the area under 95%CI:0.77~0.88), SROC curve was 0.892), and the ratio of diagnosis to diagnosis was 22.62%. Conclusion T-SPOT.TB method is helpful to improve the diagnosis rate of tuberculosis meningitis. The diagnostic accuracy of cerebrospinal fluid T-SPOT.TB may be higher than that of peripheral blood group.
