【摘要】：目的探索免疫缺陷相關的疫苗衍生脊髓灰質(zhì)炎(脊灰)病毒(Immunodeficiency-associated Vaccine-derived Poliovirus,i_ VDPV)病例的控制對策。方法對i _VDPV病例進行流行病學調(diào)查,定期采集糞便標本進行病毒分離和核苷酸檢測。開展病例血清中和抗體和免疫功能檢測。對病例居住地兒童進行口服脊灰減毒活疫苗(Oral Poliomyelitis Attenuated Live Vaccine,OPV)接種率調(diào)查,采集密切接觸者和周圍健康兒童糞便標本,開展病例居住地和醫(yī)院急性弛緩性麻痹病例主動搜索。結(jié)果病例最終分類診斷為Ⅲ型(Type 3)i _VDPV(i _VDPVⅢ)病例,最高變異率為3.4%。當?shù)貎和疧PV接種率96%,接觸者和健康兒童中分離到1株脊灰疫苗病毒。根據(jù)常規(guī)免疫接種率評價結(jié)果,確定部分地區(qū)開展了OPV查漏補種。結(jié)論 i _VDPV在當?shù)匚丛斐裳h(huán)。要保持高的OPV接種率,開展脊灰輸入傳播風險評估,為修訂免疫策略提供參考。
[Abstract]:Objective to explore the control strategies of vaccine-derived poliovirus (Immunodeficiency-associated Vaccine-derived Poliovirus,i_ VDPV) cases. Methods Epidemiological investigation was carried out on I _ VDPV cases. Stool samples were collected regularly for virus isolation and nucleotide detection. Serum neutralization antibody and immune function were detected. The coverage rate of oral attenuated poliomyelitis vaccine (Oral Poliomyelitis Attenuated Live Vaccine,OPV) was investigated in children living in their place of residence.
