【摘要】：目的：探討腸結(jié)核、肝結(jié)核的發(fā)病原因、臨床特征、影像學(xué)檢查、鑒別診斷等,提高腸結(jié)核和肝結(jié)核的診斷和治療水平。 方法：分析1例腸結(jié)核合并肝結(jié)核、結(jié)核性胸腹膜炎患者的臨床資料,結(jié)合相關(guān)文獻(xiàn)進(jìn)行復(fù)習(xí)與總結(jié)。 資料：一青年女性患者,進(jìn)行性腹脹、乏力、消瘦2月余,腹脹加重并伴有發(fā)熱、盜汗。門診體檢提示胸腹腔積液。 干預(yù)措施：行各項常規(guī)檢查及胸片、T-SPOT、腹腔穿刺、腹水TB-DNA、全腹增強CT、胃鏡、結(jié)腸鏡、婦科B超、病理檢查等以明確診斷,同時予以營養(yǎng)支持、提高免疫力及對癥治療,結(jié)合檢查結(jié)果綜合分析后予以診斷性抗結(jié)核治療,并密切觀察、隨訪。 結(jié)果：全腹CT增強示：1.右側(cè)胸腔積液并右肺下葉局限性不張,腹盆腔積液,腹膜增厚,腸系膜水腫,結(jié)合病史考慮結(jié)核性胸膜炎、結(jié)核性腹膜炎；2.肝左葉異常密度影,考慮結(jié)核可能；腸鏡示回盲部潰瘍,考慮腸結(jié)核,病檢示回盲部粘膜組織慢性炎改變。診斷性抗結(jié)核治療6周有效,最后診斷為腸結(jié)核合并肝結(jié)核、結(jié)核性胸腹膜炎。 結(jié)論：腸結(jié)核、肝結(jié)核的臨床表現(xiàn)沒有特異性,需結(jié)PPD皮試、T-SPOT、腹水TB-DNA、結(jié)腸鏡檢查、全腹增強CT、組織病理學(xué)、組織細(xì)菌學(xué)等檢查綜合判斷,可以提高診斷的準(zhǔn)確性；必要時試驗性藥物治療。
[Abstract]:Objective: to investigate the causes, clinical features, imaging examination and differential diagnosis of intestinal tuberculosis and hepatic tuberculosis, and to improve the diagnosis and treatment of intestinal tuberculosis and hepatic tuberculosis. Methods: the clinical data of a case with intestinal tuberculosis complicated with hepatic tuberculosis and tuberculous pleural peritonitis were reviewed and summarized. Data: a young female patient with progressive abdominal distension, fatigue, wasting for more than 2 months, aggravated abdominal distension with fever, night sweating. The physical examination of the outpatient department indicated hydrothorax and celiac effusion. Intervention measures: routine examination and chest film, T-SPOT, abdominal puncture, ascites TB-DNA, total abdominal enhanced CT, gastroscopy, colonoscopy, gynecological B-ultrasound, pathological examination, etc. To improve immunity and symptomatic treatment, combined with comprehensive analysis of the results of the diagnostic antituberculous treatment, and close observation, follow-up. Results: total abdominal CT showed: 1. Pleural effusion on the right side with localized atelectasis of the lower lobe of the right lung, abdominal and pelvic effusion, peritoneal thickening, mesenteric edema, tuberculous pleurisy, tuberculous peritonitis; 2. The abnormal density shadow of Zuo Ye of liver, considering the possibility of tuberculosis, and enteroscopy showing ileocecal ulcer, considering intestinal tuberculosis, showing chronic inflammation of mucosal tissue in ileocecal region. Diagnostic antituberculous treatment was effective for 6 weeks. The final diagnosis was intestinal tuberculosis with hepatic tuberculosis and tuberculous peritonitis. Conclusion: the clinical manifestations of intestinal tuberculosis and hepatic tuberculosis are not specific. It is necessary to make a comprehensive judgement on PPD skin test, T-SPOT, ascites TB-DNA, colonoscopy, total abdominal enhanced CT, histopathology, and bacteriological examination. It can improve the accuracy of diagnosis. Experimental drug therapy is necessary.
