【摘要】：目的探討獲得性免疫缺陷綜合征(AIDS)患者骨髓細(xì)胞學(xué)特征,對(duì)艾滋病的快速診斷提供依據(jù)。方法采用回顧性研究的方法,對(duì)醫(yī)院2011年1月-2012年5月278例AIDS患者骨髓細(xì)胞學(xué)檢查結(jié)果進(jìn)行統(tǒng)計(jì)分析。結(jié)果取材滿意189份,占68.0%,其中95.8%以上有核細(xì)胞增生活躍;常見細(xì)胞學(xué)異常表現(xiàn)型及分布率依次是淋巴細(xì)胞減少84.7%、紅細(xì)胞核形態(tài)異常71.4%、紅細(xì)胞緡錢狀排列59.8%、血小板減少55.0%、中性粒細(xì)胞核發(fā)育異常54.5%、粒紅比值升高45.5%、巨核細(xì)胞成熟障礙32.3%、組織細(xì)胞活化24.9%,其中淋巴細(xì)胞減少、中性粒細(xì)胞核發(fā)育異常、紅細(xì)胞核形態(tài)異常、紅細(xì)胞緡錢狀排列和血小板分布減少的比率與其他表現(xiàn)型比較差異有統(tǒng)計(jì)學(xué)意義(P0.05);在取材滿意病例中,鏡檢檢出馬爾尼菲青霉菌(penicillium marneffei,PM)陽性36例、淋巴瘤細(xì)胞2例。結(jié)論 AIDS患者的骨髓細(xì)胞學(xué)特點(diǎn)主要表現(xiàn)為粒細(xì)胞、紅細(xì)胞、巨核細(xì)胞系統(tǒng)一系或多系病態(tài)造血,淋巴細(xì)胞比率減低,常伴組織細(xì)胞反應(yīng)性增殖或活化,南方地區(qū)患者于涂片中較易見PM。
[Abstract]:Objective to investigate the characteristics of bone marrow cytology in patients with acquired immunodeficiency syndrome (AIDS) and to provide evidence for rapid diagnosis of AIDS. Methods the results of bone marrow cytology in 278 patients with AIDS from January 2011 to May 2012 were analyzed retrospectively. Results 189 samples were obtained, accounting for 68.0%, of which more than 95.8% had active proliferation of nucleated cells. The common cytological abnormal phenotypes and distribution rates were lymphocytopenia 84.7, red cell nucleus 71.4, erythrocyte roulein 59.8, thrombocytopenia 55.0, erythrocyte roulein 59.8, thrombocytopenia 55.0. The ratio of neutrophil nucleus to neutrophils was 54.5%, the ratio of granulocyte to red was increased 45.5%, the megakaryocyte maturation disorder was 32.3%, the activation of histocyte was 24.9%, in which lymphocytes were decreased, and the nucleus of neutrophil was abnormal. The ratio of erythrocyte karyotype abnormality, erythrocyte roulein arrangement and thrombocytopenia were significantly different from those of other phenotypes (P0.05). Among the satisfactory cases, 36 cases were positive for Penicillium marneffei (penicillium marneffei,PM) and 2 cases were lymphoma cells. Conclusion the main features of bone marrow cytology in AIDS patients are granulocyte, erythrocyte, megakaryocyte system with one or more lineage of morbid hematopoiesis, and the ratio of lymphocytes is decreased, which is often accompanied by reactive proliferation or activation of histocyte. PM. is more easily seen in smears of patients in southern China
