HBV基因變異對熒光定量PCR系統(tǒng)檢測HBV-DNA準(zhǔn)確性的影響

【摘要】:目的探討乙型肝炎病毒(HBV)基因型和病毒變異對Roche公司COBAS AmpliPrep/COBAS TaqMan 48系統(tǒng)和熒光定量PCR系統(tǒng)檢測HBV-DNA的影響。方法收集醫(yī)院肝病中心2014年3月-2015年10月HBV感染患者100例,明確HBV基因型和變異情況,同時(shí)采用Roche公司COBAS AmpliPrep/COBAS TaqMan48系統(tǒng)和熒光定量PCR系統(tǒng)檢測HBV-DNA,比較針對不同HBV基因型和病毒變異時(shí)兩種方法檢測結(jié)果的差異和相關(guān)性。結(jié)果兩種方法檢測B型、C型患者的病毒載量差異均無統(tǒng)計(jì)學(xué)意義,對未發(fā)生病毒變異的患者,兩種方法的檢測結(jié)果差異無統(tǒng)計(jì)學(xué)意義;對發(fā)生病毒變異的患者,兩種方法的檢測結(jié)果差異有統(tǒng)計(jì)學(xué)意義(t=2.29,P=0.028);兩種方法檢測B、C型HBV-DNA時(shí)的相關(guān)系數(shù)r分別為0.82、0.86,差異無統(tǒng)計(jì)學(xué)意義;兩種方法檢測未發(fā)生病毒變異患者HBV-DNA的相關(guān)系數(shù)r為0.91,顯著高于發(fā)生病毒變異時(shí)的相關(guān)系數(shù)(r=0.70)(Z=3.13,P0.01)。結(jié)論對于不同的HBV基因型,Roche公司COBAS AmpliPrep/COBAS TaqMan系統(tǒng)和國產(chǎn)熒光定量PCR系統(tǒng)在檢測HBV-DNA方面有較好的相關(guān)性,但HBV基因變異可能會(huì)影響后者的準(zhǔn)確性。
[Abstract]:Objective to investigate the effect of hepatitis B virus (HBV) genotype and virus variation on the detection of HBV-DNA by Roche COBAS AmpliPrep/COBAS TaqMan 48 system and fluorescence quantitative PCR system. Methods 100 patients with HBV infection from March 2014 to October 2015 were collected from Hepatology Center of Hospital. The genotype and variation of HBV were determined. Meanwhile, HBV-DNA, was detected by Roche COBAS AmpliPrep/COBAS TaqMan48 system and fluorescence quantitative PCR system. The differences and correlations between the two methods for different HBV genotypes and virus mutation were compared. Results there was no significant difference in viral load between the two methods in patients with type B and type C, but there was no significant difference between the two methods in the patients without virus variation. There was a significant difference between the two methods in the detection of HBV-DNA (t = 2.29, P 0.028), and the correlation coefficient (r) of the two methods for detecting HBV-DNA of type C was 0.82 鹵0.86, respectively, and there was no significant difference between the two methods. The correlation coefficient (r = 0.91) between the two methods for the detection of HBV-DNA in patients without virus mutation was significantly higher than that at the time of virus mutation (r = 0.70) (P 0.01). Conclusion for different HBV genotypes, Roche COBAS AmpliPrep/COBAS TaqMan system and domestic fluorescent quantitative PCR system have good correlation in HBV-DNA detection, but HBV gene variation may affect the accuracy of the latter.
