【摘要】：目的評價全血γ干擾素(IFN-γ)釋放試驗(QFT-GIT)診斷結(jié)核病的臨床應用價值。方法收集天津醫(yī)科大學總醫(yī)院2014年10月—2015年10月109例(確診病例45例,臨床診斷病例64例)結(jié)核病患者(結(jié)核組)和70例非結(jié)核呼吸系統(tǒng)疾病患者(非結(jié)核組)資料,同時采用QFT-GIT、膠體金抗結(jié)核抗體(TB-Ab)進行檢測,繪制IFN-γ釋放量受試者工作者特征(ROC)曲線,比較2種方法的診斷價值及QFT-GIT試驗在2組中的差異。結(jié)果QFT-GIT對結(jié)核病的診斷敏感度為93.58%,特異度為85.71%,其診斷陽性率高于TB-Ab(χ2=43.68,P0.01)。2種方法聯(lián)合檢測的敏感度降至52.29%(57/109),但特異度增加至90.00%(63/70)。結(jié)核組IFN-γ釋放量明顯高于非結(jié)核組(U=330,P0.05),IFN-γ釋放量ROC曲線下面積為0.913(95%CI:0.864~0.963)。結(jié)論 QFT-GIT試驗在結(jié)核病診斷中具有較高的敏感度和特異度,與TB-Ab聯(lián)合檢測可進一步提高診斷的特異度,為臨床結(jié)核病的診斷提供及時有效的依據(jù)。
[Abstract]:Objective to evaluate the clinical value of whole blood interferon 緯 (IFN- 緯) release test (QFT-GIT) in the diagnosis of tuberculosis. Methods from October 2014 to October 2015, 109 patients with tuberculosis (tuberculosis group) and 70 patients with non-tuberculosis respiratory diseases (non-tuberculosis group) were collected from Tianjin Medical University General Hospital. At the same time, QFT-GIT, colloidal gold anti-tuberculosis antibody (TB-Ab) was used to detect IFN- 緯 emission and the characteristic (ROC) curve was drawn. The diagnostic value of the two methods and the difference of QFT-GIT test in the two groups were compared. Results the sensitivity of QFT-GIT in the diagnosis of tuberculosis was 93.58 and the specificity was 85.71. The positive rate of QFT-GIT was higher than that of TB-Ab (蠂 ~ 2 / 43.68 / P0.01). The sensitivity of the two methods decreased to 52.29% (57 / 109). But the specificity increased to 90.00% (63 / 70). The release of IFN- 緯 in tuberculosis group was significantly higher than that in non-tuberculosis group (P 0.05), and the area under ROC curve of IFN- 緯 emission was 0.913 (95%CI:0.864~0.963). Conclusion QFT-GIT test has high sensitivity and specificity in the diagnosis of tuberculosis. Combined detection with TB-Ab can further improve the specificity of diagnosis and provide a timely and effective basis for the diagnosis of clinical tuberculosis.
