【摘要】：目的:觀察胸腺五肽輔助治療痰涂片鏡檢抗酸桿菌陽性(簡稱菌陽)肺結核的臨床療效及對機體免疫功能的影響。方法:選擇160例菌陽肺結核患者,隨機分為觀察組和對照組各80例,對照組采用單純化療方案治療,觀察組在對照組的基礎上聯(lián)合胸腺五肽治療,觀察兩組患者臨床療效及免疫功能改善情況。結果:治療后觀察組臨床癥狀消失情況明顯好于對照組(P0.05),不良反應發(fā)生率明顯低于對照組(P=0.036 4),痰菌轉陰率在治療后隨訪的各個時間段均明顯高于對照組(P0.05),治療總有效率明顯高于對照組(P=0.000 0);治療后觀察組CD3~+、CD4~+和CD4~+/CD8~+水平均明顯高于治療前和治療后的對照組,而CD8~+水平均明顯低于治療前和治療后的對照組(P0.05);兩組患者均未發(fā)生明顯的不良反應。結論:胸腺五肽輔助治療菌陽肺結核可顯著增強患者免疫功能,改善患者臨床癥狀與體征,提高臨床治療效果,值得臨床推廣使用。
[Abstract]:Objective: to observe the clinical effect of thymus pentapeptide in the treatment of sputum smear positive acid-fast bacilli positive pulmonary tuberculosis and its effect on immune function. Methods: 160 patients with bacillus positive pulmonary tuberculosis were randomly divided into two groups: the observation group (n = 80) and the control group (n = 80). The control group was treated with chemotherapy alone, and the observation group was treated with thymus pentapeptide on the basis of the control group. The clinical effect and immune function of the two groups were observed. Results: after treatment, the clinical symptoms disappeared significantly better in the observation group than in the control group (P0.05), and the incidence of adverse reactions was significantly lower than that in the control group (P0.0364). The sputum negative rate was significantly higher in the observation group than in the control group during the follow-up period (P0.05). The effective rate was significantly higher than that of the control group (P0. 000 0), the levels of CD3~ CD4 ~ and CD4~ / CD8 ~ in the observation group were significantly higher than those in the control group before and after treatment. However, the level of CD8~ was significantly lower than that of the control group before and after treatment (P0.05), and no significant adverse reactions occurred in both groups. Conclusion: the adjuvant treatment of bacterium-positive pulmonary tuberculosis with thymic pentapeptide can significantly enhance the immune function, improve the clinical symptoms and signs, and improve the clinical therapeutic effect, which is worth popularizing in clinic.
