【摘要】：[目的]了解泌尿系結核的診斷和治療現(xiàn)狀,并對各種手術方式療效、服藥方法療效進行簡單的探討。[方法]選取2014年～2016年間昆明醫(yī)科大學第二附屬醫(yī)院收治的82例泌尿系結核結核患者。對其發(fā)病情況、臨床癥狀、影像學資料、實驗室資料進行回顧性研究,對手術置管、服藥效果進行了評估。[結果]發(fā)病高峰年齡段為20～40歲,鄉(xiāng)村患者占86. 59%,未接種卡介苗患者占90. 24%。農(nóng)民及無業(yè)者占65. 85%。臨床癥狀依次為尿頻56 (68.29%)例、尿急41 (50.00%)例、尿痛40 (48.78%)例、腰痛35 (42.68%)例、血尿24 (29. 27%)、發(fā)熱 23 (28. 04%)例、盜汗 20 (25. 00%)。肺結核史 24 例(29. 27%)。CT檢查80例,明確診斷73例,提示異常80例。γ干擾素釋放試驗34例,32例明確診斷。76例有手術干預,余藥物治療。置管后行腎切除術9例,未置管腎切除術39例,P0. 05;[結論]腎結核好發(fā)年齡為20歲≤年齡≤40歲,且與經(jīng)濟條件、生存環(huán)境差、醫(yī)療衛(wèi)生資源缺乏有關。臨床癥狀尿頻、尿急、尿痛最為常見。肺結核史或其他部位結核仍然可以為泌尿系結核的診斷提供重要線索。B超、KUB+IVP仍然在泌尿系結核的初篩、診斷中有意義,而CT明顯優(yōu)于上述兩種檢查。γ干擾素釋放試驗臨床運用前景可觀。藥物治療在泌尿系結核治療中必不可少,而本地區(qū)用藥相對不規(guī)則。輸尿管支架置入術可以降低腎切除率�；紓饶I臟切除術,仍然是晚期腎結核的主要治療手段,首選后腹腔鏡下腎切除術。
[Abstract]:Objective: to investigate the current situation of diagnosis and treatment of urinary tuberculosis, and to discuss the curative effect of various surgical methods and medication methods.
