【摘要】：目的探討sCD163在結(jié)核性腦膜炎患者外周血漿及腦脊液中的表達(dá)水平及其對(duì)臨床診斷與預(yù)后判斷的指導(dǎo)意義。方法收集23例結(jié)核性腦膜炎患者,8例病毒性腦膜炎和6例正常人的外周血及腦脊液,ELISA法檢測(cè)血漿和腦脊液中sCD163的表達(dá),并對(duì)腦脊液進(jìn)行常規(guī)生化檢查。結(jié)果與病毒性腦膜炎和正常人相比,結(jié)核性腦膜炎患者外周血及腦脊液中sCD163表達(dá)水平明顯升高(P0.01),而病毒性腦膜炎患者與頭痛患者sCD163水平無(wú)明顯差異(P0.05)。腦脊液sCD163濃度為89.1 ng/L可作為診斷結(jié)核性腦膜炎的切點(diǎn)(cut-off)值,其敏感性和特異性分別為82.6%和93.9%。結(jié)核性腦膜炎患者腦脊液中sCD163表達(dá)與腦脊液壓力和蛋白質(zhì)含量呈正相關(guān),與腦脊液細(xì)胞數(shù)無(wú)相關(guān)性。結(jié)論 sCD163在結(jié)核性腦膜炎患者外周血和腦脊液中表達(dá)上調(diào),并且腦脊液sCD163可作為結(jié)核性腦膜炎診斷和預(yù)后判斷的生物學(xué)標(biāo)志物。
[Abstract]:Objective to investigate the expression of sCD163 in peripheral blood plasma and cerebrospinal fluid (CSF) of patients with tuberculous meningitis and its significance in clinical diagnosis and prognosis. Methods the expression of sCD163 in plasma and cerebrospinal fluid (CSF) was detected by Elisa in peripheral blood and cerebrospinal fluid of 23 patients with tuberculous meningitis and 8 patients with viral meningitis and 6 normal controls. Results compared with viral meningitis and normal subjects, the expression of sCD163 in peripheral blood and cerebrospinal fluid of patients with tuberculous meningitis was significantly higher than that of patients with viral meningitis, while the level of sCD163 in patients with viral meningitis was not significantly different from that in patients with headache. CSF sCD163 concentration of 89.1 ng/L could be used as cut-off value for diagnosis of tuberculous meningitis. Its sensitivity and specificity were 82.6% and 93.9%, respectively. The expression of sCD163 in CSF of tuberculous meningitis was positively correlated with CSF pressure and protein content, but had no correlation with the number of CSF cells. Conclusion the expression of sCD163 is up-regulated in peripheral blood and cerebrospinal fluid of patients with tuberculous meningitis, and cerebrospinal fluid sCD163 can be used as a biological marker for diagnosis and prognosis of tuberculous meningitis.
