【摘要】：目的探索老年人群中開(kāi)展主動(dòng)篩查肺結(jié)核患者的可行性。方法在江西省貧困地區(qū)采取隨機(jī)整群抽樣的方法選取21個(gè)村(居委會(huì))開(kāi)展老年人結(jié)核病患病率篩查,描述性分析老年人中結(jié)核病患病情況及影響因素,并采用Logistics單因素和多因素回歸模型擬合分析老年人患肺結(jié)核的可能危險(xiǎn)因素。結(jié)果 5 268例老年人中共篩查出21例新發(fā)活動(dòng)性肺結(jié)核患者和12例肺癌患者,60歲及以上老年人活動(dòng)性肺結(jié)核患病率為398.6/10萬(wàn),涂陽(yáng)肺結(jié)核患者患病率為132.9/10萬(wàn)。單因素分析顯示老年人體質(zhì)指數(shù)(BMI)和既往史是其結(jié)核病患病的危險(xiǎn)因素(P0.05),多因素分析顯示影響老年人結(jié)核病患病的危險(xiǎn)因素為肺結(jié)核既往史(P0.05)。結(jié)論對(duì)既往有結(jié)核病病史老年人開(kāi)展結(jié)核病篩查有積極意義。
[Abstract]:Objective to explore the feasibility of active pulmonary tuberculosis screening among the elderly. Methods 21 villages (neighborhood committees) were selected by random cluster sampling in poor areas of Jiangxi Province to screen the prevalence rate of tuberculosis among the elderly. Descriptive analysis of the prevalence of tuberculosis in the elderly and its influencing factors, Logistics single factor and multivariate regression models were used to fit and analyze the possible risk factors of pulmonary tuberculosis in the elderly. Results A total of 21 newly active pulmonary tuberculosis patients and 12 lung cancer patients aged 60 years were screened out in 5 268 elderly patients. The prevalence of active pulmonary tuberculosis in the elderly is 398.6 / 100,000. The prevalence rate of smear positive pulmonary tuberculosis patients was 132.9 / 100,000,000. Univariate analysis showed that the body mass index (BMI) and past history were the risk factors of tuberculosis in the elderly, and multivariate analysis showed that the risk factors affecting tuberculosis in the elderly were lung. Conclusion it is of positive significance for the elderly with tuberculosis history to carry out tuberculosis screening.
