青海地區(qū)藏族人群HBV易感基因IL-18的多態(tài)性研究

【摘要】:目的:通過檢測青海藏族人群乙肝患者血清中白細胞介素18(IL-18)的基因多態(tài)性,探討藏族人群乙肝高發(fā)的原因和白細胞介素18(IL-18)在HBV易感性中的作用,并為乙肝病毒攜帶者早期的診斷、排查及對乙肝患者的治療提供的理論依據(jù)。 方法:選擇本地藏族常駐人口,抽取250例乙肝患者全血5ml,根據(jù)乙肝五項的檢測結(jié)果,選擇與HBV感染者的年齡、性別匹配的134例健康者進行對照,針對IL-18的多態(tài)性位點,應(yīng)用PCR-SSCP方法篩選其在藏族人群中的分布頻率,最終通過基因測序給予鑒定;從篩選出來的IL-18基因進行統(tǒng)計學(xué)分析,計算藏族HBV感染者和健康人群中IL-18的分布頻率,判斷有無明顯差異,比較分析SNP等位基因和各個血清型的相關(guān)關(guān)系。所獲得的數(shù)據(jù)運用SPSS17.0軟件進行統(tǒng)計分析:基因型頻率及等位基因頻率的比較、估算相對危險度。 結(jié)果:1.-137G/C:病例組與對照組(1)三種基因型頻率比較:X~2值1.079,P0.05。等位基因頻率比較:X~2值0.123,P0.05。(2)logistic多因素分析:GG型OR值0.953,95%CI區(qū)間為0.517~1.757;CG型OR值0.980,95%CI區(qū)間為0.529~1.816;等位基因C型OR值1.110,95%CI區(qū)間為0.619~1.990。2.-607C/A:病例組與對照組(1)三種基因型頻率比較:X~2值1.150,P0.05。等位基因頻率比較:X~2值1.139,P0.05。(2)logistic多因素分析::CC型OR值1.169,95%CI區(qū)間為0.747~1.830; CA型OR值1.046,95%CI區(qū)間為0.685~1.598;)AA型OR值0.772,95%CI區(qū)間為0.471~1.267;等位基因A型OR值1.176,95%CI區(qū)間為0.873~1.585。3.-105A/C:病例組與對照組(1)三種基因型頻率比較:X~2值1.615,,P0.05。等位基因頻率比較:X~2值1.615,P0.05。(2)logistic多因素分析:AA型OR值0.925,95%CI區(qū)間為0.510~1.679;AC型OR值0.981,95%CI區(qū)間為0.537~1.793;等位基因C型OR值1.166,95%CI區(qū)間為0.663~2.053。 結(jié)論:IL-18的基因啟動子-607C/A、-137G/C和基因編碼區(qū)-105A/C基因多態(tài)性與乙肝易感性無明顯相關(guān)性。
[Abstract]:Objective: to investigate the cause of high incidence of hepatitis B and the role of IL-18 in susceptibility to HBV by detecting the polymorphism of interleukin-18 (IL-18) gene in serum of Tibetan patients with hepatitis B in Qinghai, and to explore the causes of the high incidence of hepatitis B and the role of IL-18 in the susceptibility of hepatitis B in Tibetan population. It also provides theoretical basis for early diagnosis, screening and treatment of hepatitis B carriers. Methods: the whole blood of 250 patients with hepatitis B was collected from the resident population of local Tibetans. According to the test results of five items of hepatitis B, 134 healthy persons matched with age and sex of HBV infection were selected as controls, and the polymorphic loci of IL-18 were analyzed. PCR-SSCP method was used to screen the distribution frequency of IL-18 in Tibetan population, and the distribution frequency of IL-18 in Tibetan HBV infected and healthy population was calculated by statistical analysis of IL-18 gene. The correlation between SNP allele and each serotype was compared and analyzed. The data obtained were statistically analyzed by SPSS17.0 software: comparing genotype frequency and allele frequency to estimate relative risk. Results: 1: 1.-137G / C: case group compared with the control group (1) the ratio of the three genotypes: 1: X2 was 1.079 / P0.05. Conclusion the gene promoter -607C / Agn-137G / C and the gene coding region -105A / C polymorphism of the gene of 1: IL-18 have no significant correlation with the susceptibility to hepatitis B.
