【摘要】：目的:觀察復(fù)方鱉甲軟肝片聯(lián)合恩替卡韋片對(duì)慢性乙型肝炎瘀血阻絡(luò)證型患者肝功能及膽紅素的影響。方法:選取2015年1-12月內(nèi)江市第二人民醫(yī)院、西南醫(yī)科大學(xué)附屬醫(yī)院、宜賓市第一人民醫(yī)院收治的110例慢性乙型肝炎瘀血阻絡(luò)證型患者,按分層區(qū)組隨機(jī)方法分為對(duì)照組和觀察組,各55例。對(duì)照組患者給予恩替卡韋片0.5 mg,po,qd;觀察組患者在此基礎(chǔ)上給予復(fù)方鱉甲軟肝片2.0 g,po,tid。兩組患者均連續(xù)治療12個(gè)月。觀察兩組患者治療前后中醫(yī)證候評(píng)分,肝功能指標(biāo)[包括天冬氨酸轉(zhuǎn)氨酶(AST)、丙氨酸轉(zhuǎn)氨酶(ALT)、堿性磷酸酶(ALP)和谷氨酰轉(zhuǎn)移酶(GGT)]、血清膽紅素[包括總膽紅素(TBIL)、直接膽紅素(DBIL)和間接膽紅素(IBIL)]、肝纖維化指標(biāo)[包括層粘連蛋白(LN)、透明質(zhì)酸(HA)、Ⅲ型前膠原(PⅢP)和Ⅳ型膠原蛋白(Ⅳ-C)]和肝瞬時(shí)彈性硬度(FS)水平,并記錄不良反應(yīng)發(fā)生情況。結(jié)果:觀察組脫落2例,對(duì)照組脫落3例。治療前,兩組患者中醫(yī)證候評(píng)分、肝功能指標(biāo)、血清膽紅素、肝纖維化指標(biāo)和FS水平比較,差異均無統(tǒng)計(jì)學(xué)意義(P0.05)。治療后,兩組患者中醫(yī)證候評(píng)分和TBIL、IBIL、LN、HA、PⅢP、Ⅳ-C、FS水平均明顯降低,DBIL水平明顯升高,且觀察組明顯優(yōu)于對(duì)照組,差異均有統(tǒng)計(jì)學(xué)意義(P0.05);觀察組患者AST、ALT水平明顯降低,ALP、GGT水平明顯升高,且觀察組明顯優(yōu)于對(duì)照組,差異均有統(tǒng)計(jì)學(xué)意義(P0.05)。觀察組患者不良反應(yīng)發(fā)生率(9.09%)與對(duì)照組(10.91%)比較,差異無統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:復(fù)方鱉甲軟肝片聯(lián)合恩替卡韋片治療慢性乙型肝炎瘀血阻絡(luò)證型患者,可改善其肝功能指標(biāo)及膽紅素水平,抑制肝纖維化,且安全性較好。
[Abstract]:Objective: to observe the effect of compound Biejia Ruanggan tablets combined with entecavir tablets on liver function and bilirubin in patients with chronic hepatitis B syndrome of blood stasis and collaterals. Methods: the second people's Hospital of Neijiang City, affiliated Hospital of Southwest Medical University, 2015 was selected. 110 patients with chronic hepatitis B syndrome of blood stasis and collaterals were randomly divided into control group and observation group according to stratified group. 55 patients in each group were treated with entecavir 0.5 mg poqd, and the patients in observation group were treated with compound Biejia Ruangan tablet 2.0 g potid. The two groups were treated continuously for 12 months. The TCM syndromes scores before and after treatment were observed in both groups.
