Fgl2通過調(diào)節(jié)巨噬細(xì)胞表型促進(jìn)肝纖維化進(jìn)展

【研究背景及目的】我國是慢性肝病發(fā)病率較高的國家之一。盡管在過去數(shù)十年,隨著乙肝疫苗的普及推廣、抗病毒治療的進(jìn)展以及醫(yī)療衛(wèi)生條件的改善,病毒性肝炎的感染率有所下降,但肝炎造成死亡的人數(shù)卻呈增加趨勢,其中大部分病毒性肝炎的死亡原因是慢性肝病,尤其是肝硬化。目前我國仍然面臨肝病這一公共衛(wèi)生問題的重大挑戰(zhàn),因此,深入研究肝纖維化肝硬化的發(fā)病機(jī)制,為其基礎(chǔ)研究和臨床治療提供新的思路和手段至關(guān)重要。肝纖維化的本質(zhì)是肝臟對慢性損傷的修復(fù)反應(yīng),其形成機(jī)制十分復(fù)雜,涉及多種細(xì)胞參與,包括受損肝細(xì)胞、肝臟非實(shí)質(zhì)細(xì)胞、募集的免疫細(xì)胞,以及細(xì)胞自分泌和旁分泌構(gòu)成的復(fù)雜的細(xì)胞因子網(wǎng)絡(luò)和信號通路。肝纖維化如未在疾病早期加以干預(yù),肝臟反復(fù)損傷后的修復(fù)反應(yīng)可導(dǎo)致細(xì)胞外基質(zhì)(extracellular matrix,ECM)過度沉積、瘢痕形成,最終進(jìn)展為肝硬化甚至肝癌。研究已證實(shí),肝星狀細(xì)胞(hepatic stellate cells,HSCs)活化在肝纖維化的發(fā)生發(fā)展中占據(jù)中心地位,持續(xù)活化的HSCs通過增殖、趨化、收縮及膠原合成,并募集更多炎癥細(xì)胞至損傷部位,放大炎癥反應(yīng),進(jìn)一步造成肝臟損傷、肝臟功能異常、肝內(nèi)...
