肺炎型肺腺癌患者EGFR基因突變和ALK基因重排情況的臨床分析
[Abstract]:Objective to investigate the characteristics and clinical significance of EGFR gene mutation and ALK gene rearrangement in patients with pneumonic lung adenocarcinoma. Methods 154 patients with lung adenocarcinoma diagnosed by histopathology in Xinqiao Hospital of the third military Medical University from January to August 2016 were selected as the study subjects. According to the imaging findings, the patients were divided into pneumonia type (30 cases). EGFR gene was detected in 154 patients, and ALK gene rearrangement was detected in 87 patients at the same time. To compare the relationship of EGFR gene mutation rate, ALK gene rearrangement rate and clinical features between pneumonic and non-pneumonic lung adenocarcinoma patients. Results the mutation rate of EGFR gene was 20.0% (6 / 30) in pneumonia type group and 47.6% (59 / 124) in non-pneumonia type group (P0.05). There was no significant difference in age, smoking history, sex, cancer family history, ALK gene rearrangement and TNM stage between the two groups (P0.05). The total mutation rate of EGFR gene was 42.2% (65 / 154). Smoking history, sex and EGFR gene mutation were related to age, tumor family history and EGFR gene mutation. The overall ALK gene rearrangement rate was 11.5%. There was no significant correlation between smoking history, family history of cancer, sex, age and ALK gene rearrangement. No mutation of EGFR gene was found in 87 patients who underwent EGFR and ALK gene detection at the same time. The co-existence of ALK gene rearrangement. Conclusion EGFR gene mutation and ALK gene rearrangement should be detected simultaneously in lung adenocarcinoma patients with pneumonic manifestations in order to establish a comprehensive management scheme for advanced tumor patients.
