不同劑量氯胺酮對體外循環(huán)缺血再灌注心肌銅鋅超氧化物歧化酶基因表達(dá)的影響

【摘要】:目的探討體外循環(huán)(CPB)手術(shù)中氯胺酮(KET)在缺血再灌注心肌對銅鋅超氧化物歧化酶(CuZn SOD)基因表達(dá)的影響及其作用機(jī)制。方法選取2006年3-9月在甘肅省人民醫(yī)院心外科行先天性心臟病修補(bǔ)手術(shù)的30例患者為研究對象,隨機(jī)分為A、B、C組(各10例),B組和C組分別靜脈滴注1 mg/kg和0.5 mg/kg的KET,A組給予同等量0.9%氯化鈉溶液干預(yù)。于主動脈阻斷前、主動脈阻斷后1h、主動脈開放后30 min從右心耳分別提取心肌組織,提取的心肌組織總核糖核酸(RNA)通過熒光定量反轉(zhuǎn)錄-聚合酶鏈反應(yīng)(RT-PCR),以β肌動蛋白(β-actin)為內(nèi)參對照Cu-Zn SOD mRNA的表達(dá)水平進(jìn)行檢測。結(jié)果 3組在主動脈阻斷前心肌組織CuZn SOD mRNA比較,差異無統(tǒng)計學(xué)意義(P0.05);A組在主動脈阻斷后1 h及主動脈開放后30 min心肌組織CuZn SOD mRNA低于B組和C組,差異有統(tǒng)計學(xué)意義(P0.05);B組和C組在主動脈阻斷后1 h及主動脈開放后30min心肌組織Cu-Zn SOD mRNA比較,差異無統(tǒng)計學(xué)意義(P0.05)。結(jié)論 KET可提高缺血再灌注心肌組織中Cu-Zn SOD mRNA含量,抑制大量氧自由基生成,從而發(fā)揮抗氧化、抗炎作用,進(jìn)而保護(hù)心肌。
[Abstract]:Objective to investigate the effect of ketamine (KET) on the expression of copper and zinc superoxide dismutase (CuZn SOD) during cardiopulmonary bypass (CPB) and its mechanism. Methods Thirty patients with congenital heart disease were selected from March to September, 2006, who underwent congenital heart repair surgery in the Department of Cardiac surgery, people's Hospital of Gansu Province. The rats were randomly divided into two groups: group B (n = 10) and group C (n = 10) treated with the same amount of sodium chloride solution for 1 mg/kg and 0.5 mg/kg respectively. Myocardial tissue was extracted from the right atrial appendage 30 min after aortic occlusion and 1 hour after aortic occlusion. Total ribonucleic acid (RNA) was extracted from myocardial tissue and the expression level of Cu-Zn SOD mRNA was detected by fluorescence quantitative reverse transcription-polymerase chain reaction (RT-PCR) and 尾 -actin as internal control. Results there was no significant difference in CuZn SOD mRNA between the three groups before aortic occlusion (P0.05). The CuZn SOD mRNA in myocardial tissue in group A was lower than that in group B and C at 1 h after aortic occlusion and 30 min after aortic opening. The difference was statistically significant (P0.05). There was no significant difference in Cu-Zn SOD mRNA between group B and group C at 1 hour after aortic occlusion and in myocardial tissue of 30min after aortic opening (P0.05). Conclusion KET can increase the content of Cu-Zn SOD mRNA and inhibit the production of oxygen free radicals in ischemic reperfusion myocardium, thus exerting anti-oxidation and anti-inflammatory effects, and then protecting myocardium.
