【摘要】：目的:采用Meta分析的方法評估前列腺干細胞抗原(prostate stem cell antigen,PSCA)基因rs2294008 CT位點多態(tài)性和東亞人群罹患胃癌風險的相關(guān)性。方法:計算機檢索PubMed、Web of Science、Cochrane Library、Google學術(shù)、中國知網(wǎng)、中國生物醫(yī)學文獻數(shù)據(jù)庫、維普和萬方等各大數(shù)據(jù)庫,收集建庫開始至2016年6月6日公開發(fā)表的關(guān)于PSCA基因rs2294008位點多態(tài)性與東亞人群罹患胃癌風險的病例-對照研究。按照納入及排除標準篩選文獻,進行質(zhì)量評價后提取數(shù)據(jù)。采用STATA 14.1軟件進行Meta分析,計算合并比值比(odds ratio,OR)及95%可信區(qū)間(confidence interval,CI),并進行亞組分析和Meta回歸分析以評估混雜因素,最后進行敏感性分析和發(fā)表偏倚檢測。結(jié)果:共納入13項病例-對照研究,包含13 667例胃癌患者和11 868例健康對照。Meta分析顯示,與其他基因型相比,東亞人群攜帶PSCA rs2294008CT和TT基因型時罹患胃癌的風險明顯增高(OR=1.68,95%CI:1.43~1.96);PSCA rs2294008多態(tài)性在中國、日本和韓國人群之間存在顯著的分布差異(P=0.001)。亞組分析提示,攜帶PSCA rs2294008 CT和TT基因型的中國人群(OR=1.36,95%CI:1.23~1.49,P=0.004)、日本人群(OR=2.61,95%CI:2.22~3.07,P=0.001)和韓國人群(OR=2.04,95%CI為1.28~3.27,P=0.012)罹患胃癌的風險比其他基因型均明顯增高;按對照來源為醫(yī)院和社區(qū)進行的亞組分析顯示,各組別罹患胃癌的風險無明顯差異(OR=1.74,95%CI:1.23~2.44;OR=1.64,95%CI:1.40~1.93)。Meta回歸分析顯示,國家和對照組來源的差異不是納入研究之間異質(zhì)性的主要來源(P=0.163,P=0.381)。Begg檢測未發(fā)現(xiàn)發(fā)表偏倚(校正P=0.053),敏感性分析確認上述結(jié)果具有穩(wěn)定性(OR=1.54,95%CI:1.45~1.64)。結(jié)論:PSCA rs2294008 CT基因多態(tài)性與東亞人群罹患胃癌的風險密切相關(guān)。
[Abstract]:Objective: To evaluate the correlation of the rs2294008 CT site polymorphism of the prostate stem cell antigen (prostate stem cell antigen, PSCA) gene and the risk of gastric cancer in East Asia by Meta analysis. A case-control study on the polymorphism of the PSCA gene rs2294008 locus and the risk of gastric cancer in East Asia, published in June 6, 2016, was collected from the large databases such as universal and Wanfang. The data were extracted after quality evaluation according to the inclusion and exclusion criteria, and the Meta analysis was carried out by STATA 14.1 software. Combined ratio Ratio (odds ratio, OR) and 95% confidence interval (confidence interval, CI), subgroup analysis and Meta regression analysis were used to evaluate confounding factors. Finally, sensitivity analysis and publication bias detection were carried out. Results: a total of 13 case control studies were included, including 13667 cases of gastric cancer and 11868 healthy control.Meta analysis, and The risk of gastric cancer in the East Asian population with PSCA rs2294008CT and TT genotype was significantly higher than other genotypes (OR=1.68,95%CI:1.43~1.96), and there was a significant distribution difference between the PSCA rs2294008 polymorphism in China and the Korean population (P=0.001). The subgroup analysis suggested that the Chinese people carrying PSCA rs2294008 CT and TT genotype were in China. OR=1.36,95%CI:1.23~1.49 (P=0.004), Japanese population (OR=2.61,95%CI:2.22~3.07, P=0.001) and Korean population (OR=2.04,95%CI 1.28~3.27, P=0.012) were significantly higher in risk of gastric cancer than other genotypes. Subgroup analysis of hospitals and communities according to a control source showed no significant difference in the risk of gastric cancer in each group (OR=1.). 74,95%CI:1.23~2.44; OR=1.64,95%CI:1.40~1.93).Meta regression analysis showed that the difference between the source of the country and the control group was not the main source of the heterogeneity among the studies (P=0.163, P=0.381).Begg detection did not find the publication bias (correction P=0.053), the sensitivity analysis confirmed that the above results were stable (OR=1.54,95%CI:1.45~1.64). Conclusion: PSCA The polymorphism of rs2294008 CT gene is closely related to the risk of gastric cancer in East Asia.
