肺癌患者GSTP1基因啟動(dòng)子甲基化檢測及其臨床價(jià)值

【摘要】:目的在肺癌的發(fā)生發(fā)展過程中,甲基化異常是導(dǎo)致相關(guān)基因失活的重要機(jī)制。本研究探討肺癌患者谷胱甘肽S轉(zhuǎn)移酶P1基因(Glutathione S-transferases P1,GSTP1)甲基化水平及其對肺癌診斷的臨床價(jià)值。方法收集2015-10-01-2016-01-31鄭州大學(xué)第一附屬醫(yī)院收治的100例患者的肺泡灌洗液,其中80例肺癌患者(小細(xì)胞肺癌和非小細(xì)胞肺癌)為肺癌組,20例良性肺疾病患者(肺炎、慢性阻塞性肺疾病和肺結(jié)核)為對照組。以PCR聯(lián)合高分辨率熔解曲線(high resolution melting assay,HRM)(PCR-HRM)方法檢測GSTP1基因啟動(dòng)子甲基化水平,并結(jié)合肺癌患者臨床病理特征進(jìn)行分析。結(jié)果肺癌組GSTP1基因啟動(dòng)子甲基化率為42.5%(34/80),顯著高于對照組的15.0%(3/20),χ~2=5.191,P=0.023。不同年齡、性別及病理類型肺癌患者GSTP1基因啟動(dòng)子甲基化率,差異均無統(tǒng)計(jì)學(xué)意義,P值分別為0.397、0.853和0.751。早期(Ⅰ和Ⅱ期)肺癌患者GSTP1基因啟動(dòng)子甲基化率42.5%(17/40),顯著低于晚期(Ⅲ和Ⅳ期)的70.0%(28/40),χ~2=11.931,P=0.013;但顯著高于對照組,χ~2=4.538,P=0.033。高、中分化肺癌患者GSTP1基因啟動(dòng)子甲基化率為43.9%(18/41),顯著低于低分化患者的79.5%(31/39),χ~2=10.664,P=0.010;但顯著高于對照組,χ~2=4.974,P=0.026。結(jié)論 GSTP1基因啟動(dòng)子甲基化改變與肺癌發(fā)生發(fā)展及惡性程度有關(guān),GSTP1基因啟動(dòng)子甲基化檢測可能有助于肺癌診斷。
[Abstract]:Objective aberrant methylation is an important mechanism leading to gene inactivation in lung cancer. The aim of this study was to investigate the methylation level of glutathione S-transferases P1 gene in patients with lung cancer and its clinical value in the diagnosis of lung cancer. Methods the alveolar lavage fluid was collected from 100 patients admitted to the first affiliated Hospital of Zhengzhou University from 2015-10-01-2016-01-31. Among them, 80 patients with lung cancer (small cell lung cancer and non-small cell lung cancer) were divided into lung cancer group and 20 patients with benign lung disease (pneumonia). Chronic obstructive pulmonary disease (COPD) and pulmonary tuberculosis (TB) were used as control group. PCR and high resolution fusion curve were used to detect the methylation level of GSTP1 promoter in lung cancer patients. Results the methylation rate of GSTP1 gene promoter in lung cancer group was 42.5 / 80%, which was significantly higher than that in control group (15.0 / 20 / 20). The promoter methylation rate of GSTP1 gene in patients with lung cancer of different age, sex and pathological type was not statistically significant (P = 0.3970.853,0.751respectively).
