新疆漢族及維吾爾族結(jié)直腸癌原發(fā)灶與轉(zhuǎn)移灶中KRAS基因突變分析

【摘要】:目的觀察新疆地區(qū)漢族、維吾爾族人群中結(jié)直腸癌(colorectal cancer,CRC)原發(fā)灶與轉(zhuǎn)移灶中KRAS基因狀態(tài)的差異及其與CRC患者臨床病理特征之間的關(guān)系。方法收集2012年~2014年間新疆地區(qū)漢族及維吾爾族CRC標(biāo)本189例,其中52例有配對轉(zhuǎn)移灶。采用突變擴(kuò)增阻滯系統(tǒng)(amplification refractory mutation system,ARMS)進(jìn)行KRAS基因12、13密碼子已知常見突變位點(diǎn)檢測。結(jié)果KRAS基因在CRC原發(fā)灶病例中漢族、維吾爾族的突變率分別為42.8%(62/145)、43.2%(19/44),兩者差異無顯著性(P0.05),T3/T4期及有淋巴結(jié)轉(zhuǎn)移的病例中突變率較高(P0.05),但與患者性別、民族、年齡、分化程度、TNM分期、有無遠(yuǎn)處轉(zhuǎn)移、原發(fā)灶所在的部位均無相關(guān)性(P0.05)。KRAS基因在CRC轉(zhuǎn)移灶中漢族、維吾爾族的突變率分別為33.3%(12/36)、31.3%(5/16),兩者差異無顯著性(P0.05),并發(fā)現(xiàn)年齡≥65歲者突變率較高(P0.05),與所研究的其他臨床病理特征無相關(guān)性。52例轉(zhuǎn)移灶中KRAS基因的突變率為32.7%(17/52),而對應(yīng)的原發(fā)灶突變率達(dá)50%(26/52),明顯高于轉(zhuǎn)移灶(P0.000 1),轉(zhuǎn)移灶突變類型與原發(fā)灶一致。結(jié)論 KRAS基因在浸潤較深、有淋巴結(jié)轉(zhuǎn)移及65歲以上發(fā)生轉(zhuǎn)移的CRC中突變率較高,在原發(fā)灶的突變率高于配對轉(zhuǎn)移灶。KRAS基因突變率在新疆漢族、維吾爾族CRC患者中差異無顯著性,提示KRAS基因突變促進(jìn)CRC的進(jìn)展,并且在原發(fā)灶中更易發(fā)生突變,但漢族、維吾爾族間無差異。
[Abstract]:Objective to observe the difference of KRAS gene status between primary and metastatic colorectal cancer in Han and Uygur populations in Xinjiang and its relationship with clinicopathological features of CRC patients. Methods 189 CRC specimens from Han and Uygur nationalities in Xinjiang were collected from 2012 to 2014, of which 52 cases had pairing metastatic foci. The mutation amplification block system amplification refractory mutation system ARMS was used to detect the known common mutation sites of the KRAS gene codon 12n13. Results the mutation rates of KRAS gene in Han and Uygur patients with primary CRC were 42. 8 and 42 / 145, respectively. There was no significant difference between the two groups. The mutation rate of KRAS gene in patients with lymph node metastasis was higher than that in patients with lymph node metastasis (P 0. 05), but it was associated with sex, nationality, age, differentiation and TNM stage. There was no correlation between the location of the primary tumor and the distant metastasis. KRAS gene was found in the Han nationality in the CRC metastasis.
