前列腺癌ERG基因重排與蛋白表達(dá)及其與預(yù)后的關(guān)系

【摘要】:目的檢測(cè)中國(guó)前列腺癌患者ETS相關(guān)基因(ERG)重排及蛋白表達(dá)情況,分析其與患者預(yù)后的關(guān)系。方法選取四川大學(xué)華西醫(yī)院2009~2014年前列腺穿刺確診的前列腺癌患者482例,采用熒光原位雜交法檢測(cè)ERG基因重排情況,采用免疫組化染色法檢測(cè)ERG蛋白表達(dá)情況。采用Spearman秩相關(guān)分析ERG基因重排與蛋白表達(dá)的相關(guān)性,采用χ~2檢驗(yàn)分析ERG基因重排和蛋白表達(dá)與患者臨床病理特征的關(guān)系,采用KaplanMeier法分析ERG基因重排和蛋白表達(dá)與患者預(yù)后的關(guān)系。結(jié)果 87例(18.0%)患者檢出ERG基因重排,其中45例(51.7%)為轉(zhuǎn)位重排,42例(48.3%)為缺失重排。74例(15.4%)患者表達(dá)ERG蛋白。ERG蛋白表達(dá)與基因重排正相關(guān)(r=0.849,P=0.000)。368例獲得隨訪資料,ERG基因重排狀態(tài)及蛋白表達(dá)水平與患者年齡、Gleason評(píng)分、初診時(shí)前列腺特異性抗原(PSA)水平無(wú)關(guān)(P0.05),但發(fā)生遠(yuǎn)處轉(zhuǎn)移及進(jìn)展為去勢(shì)抵抗性前列腺癌(CRPC)的病例中ERG蛋白表達(dá)率下降(P0.05)。預(yù)后分析顯示ERG基因重排及蛋白表達(dá)均不能提示預(yù)后。結(jié)論 ERG基因重排狀態(tài)與蛋白表達(dá)情況均不能作為前列腺癌進(jìn)展為CRPC及總生存率的獨(dú)立預(yù)測(cè)指標(biāo)。
[Abstract]:Objective to study the relationship between ETS gene rearrangement and protein expression in Chinese patients with prostate cancer. Methods from 2009 to 2014, 482 patients with prostate cancer diagnosed by prostate puncture in Huaxi Hospital of Sichuan University were selected. The ERG gene rearrangement was detected by fluorescence in situ hybridization, and the expression of ERG protein was detected by immunohistochemical staining. Spearman rank correlation analysis was used to analyze the correlation between ERG gene rearrangement and protein expression, and 蠂 2 test was used to analyze the relationship between ERG gene rearrangement and protein expression and clinicopathological features. The relationship between ERG gene rearrangement and protein expression and prognosis was analyzed by KaplanMeier method. Results ERG gene rearrangement was detected in 87 patients. Among them, 45 cases (51.7) (42 cases with transposition rearrangement) and 74 cases with missing rearrangement (15. 4%) expressed ERG protein. ERG protein expression and gene rearrangement were positively correlated with gene rearrangement. 368 cases were followed up. The status and expression level of ERG gene rearrangement and protein expression were compared with Gleason score of patients. The expression rate of ERG protein was decreased in patients with distant metastasis and progression to castrated resistant prostate cancer (CRPC). Prognostic analysis showed that neither ERG gene rearrangement nor protein expression could predict prognosis. Conclusion neither ERG gene rearrangement nor protein expression can be used as an independent predictor of CRPC and overall survival rate in prostate cancer.
