【摘要】:目的探討HPA基因多態(tài)性及其等位基因?qū)ρ“遢斪⑿Ч挠绊憽7椒ㄗ?015年1月-2017年1月,連續(xù)性收集50例本院收治的血小板輸注無(wú)效患者作為觀察組,同期收集50例血小板輸注有效的患者作為對(duì)照組,觀察2組患者HPA基因多態(tài)性。結(jié)果與對(duì)照組比較,觀察組患者供患雙方HPA分型相合率顯著降低(4.00%vs 82.00%,P0.05);血小板恢復(fù)百分比顯著降低[(11.91±3.23)%vs(31.56±7.38)%,P0.05);血小板糾正增加指數(shù)顯著降低(6.84±2.27 vs 16.38±1.92,P0.05)。2組患者HPA等位基因(HPA1-6,9,15)均以純合子aa為多見(jiàn)。但觀察組HPA-2等位基因?yàn)閍a、ab、bb的分別有37、8、5例,對(duì)照組分別有48、2、0例,差異有統(tǒng)計(jì)學(xué)意義(P0.05);觀察組HPA-3等位基因?yàn)閍a、ab、bb的分別有33、9、8例,對(duì)照組分別有46、3、1例,差異有統(tǒng)計(jì)學(xué)差異(P0.05)。觀察組HPA-15等位基因?yàn)閍a、ab、bb的分別有35、8、7例,對(duì)照組分別有47、2、1例,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。2組患者的血小板供者HPA等位基因(HPA1-6,9,15)均以純合子aa為多見(jiàn),差異均無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。血小板輸注無(wú)效患者HPA-2,3等位基因?yàn)閍a、ab和bb的血小板恢復(fù)百分比、血小板糾正增加指數(shù)逐步下降(P0.05)。結(jié)論血小板輸注無(wú)效與HPA基因多態(tài)性有關(guān),等位基因型b可能是導(dǎo)致血小板輸注無(wú)效的原因。
[Abstract]:Objective to investigate the effect of HPA gene polymorphism and its allele on platelet transfusion. Methods from January 2015 to January 2017, 50 consecutive patients with ineffective platelet transfusion were collected as observation group and 50 patients with effective platelet transfusion as control group. The polymorphism of HPA gene was observed in both groups. Results compared with the control group, In the observation group, the consistent rate of HPA typing was significantly decreased by 4.00 vs 82.00, the percentage of platelet recovery was significantly decreased [11.91 鹵3.23)%vs(31.56 鹵7.38] and the platelet correction index was significantly decreased (6.84 鹵2.27 vs 16.38 鹵1.92 P0.05.2). However, the HPA-2 allele of the observation group was aaaababanbb (37 / 85) and the control group (48 / 22), the difference was statistically significant (P0.05), the HPA-3 allele of the observation group was aaaababambb (339 / 8) and the control group (466 / 31), respectively (P 0.05). In the observation group, the allele of HPA-15 was aaaababanbb in 37 cases and that in the control group was 47 cases. The difference between the two groups was statistically significant (P 0.05). The homozygote AA was the most common allele in the platelet donor group (P 0.05), and there was no significant difference in the difference between the two groups (P 0. 05%, P 0. 05, P 0. 05, P 0. 05, P 0. 05, P 0. 05, P 0. 05, P 0. 05, P 0. 05, P 0. 05, P 0. 05). The alleles of HPA-2M3 in patients with ineffective platelet transfusion were the percentage of platelet recovery of aaqinab and BB, and the increase index of platelet correction decreased gradually (P 0.05). Conclusion HPA gene polymorphism is associated with platelet transfusion failure, and allelic genotype b may be the cause of platelet transfusion failure.
