產(chǎn)ESBLs大腸埃希菌對氨基糖苷類藥物的耐藥性及其耐藥基因序列分析

【摘要】:目的研究云南玉溪市江川地區(qū)產(chǎn)超廣譜β-內(nèi)酰胺酶(ESBLs)大腸埃希菌對氨基糖苷類抗菌藥物的耐藥性及其耐藥基因表達(dá)的頻率,并對其耐藥基因進(jìn)行測序分析。方法采用紙片擴(kuò)散法檢測32株產(chǎn)ESBLs大腸埃希菌對慶大霉素、阿米卡星、卡那霉素、妥布霉素和奈替米星的耐藥性。采用聚合酶鏈反應(yīng)(PCR)檢測7種氨基糖苷類修飾酶(AME)基因[aac(3)-Ⅰ、aac(3)-Ⅱ、aac(6′)-Ⅰad、aac(6′)-Ⅰb、aac(6′)-Ⅱ、ant(3″)-Ⅰ、ant(2″)-Ⅰ]和6種16S rRNA甲基化酶基因(armA、rmtA、rmtB、rmtC、rmtD、npmA),采用雙脫氧末端終止法對aac(3)-Ⅱ基因進(jìn)行測序分析。結(jié)果 32株產(chǎn)ESBLs大腸埃希菌對慶大霉素、阿米卡星、奈替米星、卡那霉素和妥布霉素的耐藥率分別為100%、0%、0%、18%和87%。有6株菌株同時(shí)攜帶2種AME基因。從32株產(chǎn)ESBLs大腸埃希菌中檢出aac(3)-Ⅱ、aac(6′)-Ⅰb、ant(3″)-Ⅰ、ant(2″)-Ⅰ4種AME基因,陽性率分別為100%、9.4%、6.2%和3.1%;未檢出16S rRNA甲基化酶基因。32株產(chǎn)ESBLs大腸埃希菌中有4株發(fā)生相同的aac(3)-Ⅱ突變,分別為G232A、T251C、G580A和T612A。8株不產(chǎn)ESBLs大腸埃希菌均未檢出AME基因及16S rRNA甲基化酶基因。結(jié)論云南玉溪市江川地區(qū)產(chǎn)ESBLs大腸埃希菌耐氨基糖苷類藥物與AME基因表達(dá)有關(guān),奈替米星的耐藥率增加可能與aac(3)-Ⅱ基因突變有關(guān)。
[Abstract]:Objective to study the resistance of Escherichia coli to aminoglycoside antibiotics and its gene expression frequency in Jiangchuan area of Yuxi City Yunnan Province and to analyze its resistance genes by sequencing.
