【摘要】：目的探討老年原發(fā)性血小板增多癥(ET)患者Jak2基因v617f點(diǎn)突變檢測(cè)的臨床意義。方法老年ET患者100例,檢測(cè)Jak2基因v617f點(diǎn)突變,根據(jù)檢測(cè)結(jié)果分為陰性組(n=54)與陽性組(n=46),比較兩組年齡、性別、脾腫大、骨髓巨核細(xì)胞數(shù)、實(shí)驗(yàn)室檢查指標(biāo)及血栓發(fā)生率、出血率。結(jié)果兩組年齡、性別、脾腫大及骨髓巨核細(xì)胞數(shù)差異均無統(tǒng)計(jì)學(xué)意義(均P0.05)。陽性組白細(xì)胞計(jì)數(shù)(WBC)水平顯著高于陰性組(P0.05),兩組血紅蛋白(HGB)、血小板計(jì)數(shù)(PLT)水平比較差異無統(tǒng)計(jì)學(xué)意義(P0.05)。陽性組血栓發(fā)生率顯著高于陰性組(P0.05),兩組出血率比較差異無統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論 Jak2基因v617f點(diǎn)突變檢測(cè)有利于輔助臨床診斷老年ET,有效評(píng)估患者疾病狀態(tài),并為臨床治療和預(yù)防血栓的發(fā)生提供參考依據(jù)。
[Abstract]:Objective to investigate the clinical significance of Jak2 v617 f point mutation in elderly patients with idiopathic thrombocytopenia. Methods the point mutation of Jak2 gene v617f was detected in 100 elderly patients with et. The results were divided into negative group (n = 54) and positive group (n = 46). The age, sex, splenomegaly, bone marrow megakaryocyte number, laboratory examination index, thrombosis rate and bleeding rate were compared between the two groups. Results there was no significant difference in age, sex, splenomegaly and the number of bone marrow megakaryocytes between the two groups (P 0.05). The level of WBCC in the positive group was significantly higher than that in the negative group (P 0.05), but there was no significant difference between the two groups in the levels of HGB and PLT (P 0.05). The incidence of thrombus in the positive group was significantly higher than that in the negative group (P 0.05). There was no significant difference in the bleeding rate between the two groups. Conclusion the detection of v617f point mutation of Jak2 gene is helpful for the diagnosis of senile ETS, the evaluation of disease status, and the reference for clinical treatment and prevention of thrombus.
