【摘要】：基本醫(yī)療保險(xiǎn)作為維護(hù)健康權(quán)這一基本人權(quán)的主要制度,是一個(gè)提供起點(diǎn)公平的制度,與其他社會(huì)保險(xiǎn)不同,它的核心價(jià)值觀是"平等"。到21世紀(jì)中葉實(shí)現(xiàn)全民統(tǒng)一的、保障水平適度的基本醫(yī)療保險(xiǎn)制度已成政學(xué)兩界的共識(shí),有分歧的是關(guān)于實(shí)現(xiàn)目標(biāo)的路徑。當(dāng)前主流觀點(diǎn)和多地的試點(diǎn)政策主張的是走先合并城鄉(xiāng)居民醫(yī)保再考慮合并居民醫(yī)保和職工醫(yī)保的"居民—職工"模式�；谌司I資是否能基本實(shí)現(xiàn)均等化的模擬分析表明,此模式下居民制度和職工制度在人均籌資上的絕對(duì)差距只會(huì)越來(lái)越大,一體化之路幾無(wú)可能。如若以退為進(jìn),選擇基于常住地參保、家庭聯(lián)保、強(qiáng)制參保原則的,先實(shí)現(xiàn)城鎮(zhèn)內(nèi)部諸制度一體化,同時(shí)改革完善新農(nóng)合制度,進(jìn)而順時(shí)順勢(shì)實(shí)現(xiàn)城鄉(xiāng)制度自然融合的"城鎮(zhèn)—農(nóng)村"模式,則不僅全民基本醫(yī)保一體化的目標(biāo)可以實(shí)現(xiàn),同時(shí)還可達(dá)成優(yōu)化醫(yī)療衛(wèi)生體制、助推城鎮(zhèn)化進(jìn)程的正外部效應(yīng)。
[Abstract]:As the main system of safeguarding the right to health, the basic medical insurance is a system that provides a fair starting point. Unlike other social insurance, its core value is "equality".By the middle of the 21st century, the basic medical insurance system, which is unified by the whole people and with a moderate level of security, has become the consensus of the two circles of politics, but what is different is the way to achieve the goal.At present, the mainstream view and the pilot policy of many places advocate the "inhabitant-worker" mode of merging urban and rural residents' medical insurance first and then considering the combination of residents' medical insurance and employee's medical insurance.The simulation analysis based on whether per capita financing can basically achieve equalization shows that the absolute gap between resident system and worker system in per capita financing will only become larger and larger under this model, and the road of integration is almost impossible.If we take retreat as a step forward and choose the principle of permanent residence insurance, family protection, and compulsory insurance, we should first realize the integration of various systems within the town, and at the same time reform and perfect the new rural cooperative system.Then the "urban-rural" mode of natural integration of urban and rural systems can be realized in a timely and favorable manner, which can not only achieve the goal of the integration of basic health insurance for all people, but also achieve the positive external effects of optimizing the medical and health system and promoting the process of urbanization.
