【摘要】：自2002年10月,我國明確提出各級政府要積極引導農民建立以大病統(tǒng)籌為主的新型農村合作醫(yī)療制度后,新型農村合作醫(yī)療在全國各地迅速發(fā)展起來,參合率、籌資總額、補償比例不斷提高,合作醫(yī)療是由我國農民自己創(chuàng)造的互助共濟的醫(yī)療保障制度,它在保障農民獲得基本衛(wèi)生服務、緩解農民因病致貧和因病返貧方面發(fā)揮了重要的作用,為世界各國,特別是發(fā)展中國家所普遍存在的問題提供了一個范本,受到了農民群眾的普遍歡迎。濰坊市新型農村合作醫(yī)療制度從2001年開始探索,在發(fā)展過程中經歷了自行探索、初步發(fā)展、全面推開等三個階段,到目前為止取得了顯著成績,農民獲益頗多。盡管濰坊市新型農村合作醫(yī)療取得了一些成績,但在發(fā)展過程中,特別是在目前城鄉(xiāng)醫(yī)保并軌的過渡期,依然存在許多問題,這些問題的存在阻礙了濰坊市新型農村合作醫(yī)療效用的發(fā)揮,需要采取措施盡快切實加以解決。本文立足于濰坊市,借鑒已有的研究成果,通過調查研究,結合本人實際工作的經歷,采用比較分析、實證舉例、定量分析、系統(tǒng)歸納等方法,闡述了四種醫(yī)療保險制度模式,指出我國的醫(yī)療保險制度模式從總體特征來看屬于社會醫(yī)療保險性質,說明了我國合作醫(yī)療的發(fā)展歷程,指出新型農村合作醫(yī)療制度是在傳統(tǒng)農村合作醫(yī)療基礎上發(fā)展起來的,同時介紹了新型農村合作醫(yī)療的理論基礎,為本文的研究提供了理論依據(jù)。論文介紹了濰坊市新型農村合作醫(yī)療的主要做法、取得的成績,指出濰坊市新農合在參合率、籌資額度、補償率上都有了大幅提升,基金運行規(guī)范有效,大病保障框架基本建立,醫(yī)療救助制度發(fā)揮實效,三級醫(yī)療衛(wèi)生服務網(wǎng)絡建設飛速發(fā)展,并結合實踐說明了濰坊市新農合運行中存在的問題,包括出資主體不健全,籌資水平仍較低;定點醫(yī)療機構偏少,分布不均;職能不順,監(jiān)督機制不健全;鄉(xiāng)村衛(wèi)生基礎設施依然落后等;最后闡釋了濰坊市新型農村合作醫(yī)療的發(fā)展策略,指出濰坊市新型農村合作醫(yī)療的發(fā)展要遵循一定的原則,采取多種策略,需要在繼續(xù)深化醫(yī)藥衛(wèi)生體制改革的基礎上,進一步健全籌資主體,延伸統(tǒng)籌層次,改革補償模式,提高補償水平,加大醫(yī)療救助力度、進一步健全全市基層醫(yī)療衛(wèi)生服務網(wǎng)絡,按照《山東省整合城鄉(xiāng)居民基本醫(yī)療保險工作實施方案》的要求,逐步向城鄉(xiāng)醫(yī)保并軌過渡。
[Abstract]:Since October 2002, the government at all levels of our country has clearly proposed that the farmers should be actively guided to establish a new rural cooperative medical care system, which is mainly concerned with serious illness. After that, the new rural cooperative medical system has developed rapidly throughout the country, with participation rate and total amount of funds raised. The proportion of compensation continues to increase. Cooperative medical care is a medical security system created by farmers in our country. It plays an important role in guaranteeing farmers access to basic health services and alleviating poverty caused by illness and returning to poverty because of illness. It provides a model for the problems that exist all over the world, especially in the developing countries, and is generally welcomed by the peasants. Weifang's new rural cooperative medical system has been explored since 2001. In the process of development, we have experienced three stages: self-exploration, preliminary development and all-around development. So far, remarkable achievements have been made, and farmers have benefited a lot. Although Weifang New Rural Cooperative Medical Scheme has made some achievements, However, in the process of development, especially in the transition period of urban and rural medical insurance integration, there are still many problems, which hinder the effectiveness of the new rural cooperative medical system in Weifang. We need to take measures to solve the problem as soon as possible. This paper bases on Weifang city, draws lessons from the existing research results, through investigation and research, combining with my actual work experience, using comparative analysis, empirical examples, quantitative analysis, This paper expounds four kinds of medical insurance system models, points out that the medical insurance system mode in our country belongs to the nature of social medical insurance from the general characteristics, and explains the development course of cooperative medical system in China. It is pointed out that the new rural cooperative medical system is developed on the basis of the traditional rural cooperative medical system, and the theoretical basis of the new rural cooperative medical system is also introduced. The paper introduces the main methods of the new rural cooperative medical system in Weifang, and points out that the new rural cooperative medical system in Weifang has greatly promoted the participation rate, the financing quota and the compensation rate. The operation of the fund is standardized and effective, the guarantee framework for serious illness is basically established, the medical aid system is effective, the three-level medical and health service network is developing rapidly, and the problems existing in the operation of Weifang New Rural Cooperation are explained in combination with practice. The main body of investment is not perfect, the level of financing is still low; the number of designated medical institutions is on the low side, the distribution is uneven, the function is not good, the supervision mechanism is not perfect, the rural health infrastructure is still backward, etc. Finally, the paper explains the development strategy of the new rural cooperative medical system in Weifang, and points out that the development of the new rural cooperative medical system in Weifang should follow certain principles, adopt various strategies, and continue to deepen the reform of the medical and health system. We will further improve the main body of fund-raising, extend the overall planning level, reform the compensation model, raise the level of compensation, increase the intensity of medical assistance, and further improve the basic level medical and health service network of the city. In accordance with the requirements of the implementation Plan of integrating the basic Medical Insurance for Urban and Rural residents in Shandong Province, the transition to the integration of urban and rural medical insurance should be gradually carried out.
