基于《大醫(yī)精誠(chéng)》醫(yī)學(xué)倫理思想的護(hù)生專(zhuān)業(yè)價(jià)值觀研究

【摘要】:[目的] 本研究以《大醫(yī)精誠(chéng)》中蘊(yùn)含的醫(yī)學(xué)倫理思想為內(nèi)涵來(lái)充實(shí)護(hù)理倫理學(xué)基本框架,并以此為基礎(chǔ)構(gòu)建符合我國(guó)文化背景和基本國(guó)情的護(hù)理專(zhuān)業(yè)價(jià)值觀量表結(jié)構(gòu),進(jìn)而編制符合中國(guó)文化特點(diǎn)的護(hù)生專(zhuān)業(yè)價(jià)值觀量表,以便為我國(guó)護(hù)理教育者今后深入開(kāi)展提升護(hù)生專(zhuān)業(yè)價(jià)值觀的實(shí)證性研究提供適宜的評(píng)價(jià)工具。另一方面以本研究研制的護(hù)生專(zhuān)業(yè)價(jià)值觀量表為測(cè)量工具進(jìn)行調(diào)查,了解在校本科護(hù)生的專(zhuān)業(yè)價(jià)值觀現(xiàn)狀,并探討可能存在的影響因素。 [方法] 本研究通過(guò)回顧國(guó)內(nèi)外護(hù)理專(zhuān)業(yè)價(jià)值觀的相關(guān)文獻(xiàn),探討《大醫(yī)精誠(chéng)》中的醫(yī)學(xué)倫理思想和護(hù)理倫理學(xué)相關(guān)理論,得出本研究的理論框架。再以《大醫(yī)精誠(chéng)》中的醫(yī)學(xué)倫理思想內(nèi)涵為依據(jù)編制訪談提綱對(duì)在校護(hù)生和在職護(hù)士進(jìn)行深入訪談,從而獲得編制量表的條目進(jìn)而形成護(hù)生專(zhuān)業(yè)價(jià)值觀初始問(wèn)卷。最后采用探索性因子分析方法對(duì)數(shù)據(jù)進(jìn)行分析,獲得護(hù)生專(zhuān)業(yè)價(jià)值觀最終量表,并對(duì)其進(jìn)行信效度檢驗(yàn)。 以本研究研制的護(hù)生專(zhuān)業(yè)價(jià)值觀量表為工具對(duì)北京、天津等地三所高等醫(yī)學(xué)院校的894名護(hù)理本科生進(jìn)行調(diào)查。采用SPSS統(tǒng)計(jì)軟件對(duì)得到數(shù)據(jù)進(jìn)行統(tǒng)計(jì),進(jìn)一步驗(yàn)證新編量表的結(jié)構(gòu)擬合程度、并對(duì)護(hù)生專(zhuān)業(yè)價(jià)值觀的基本情況和可能存在的影響因素進(jìn)行分析。 [結(jié)果] 護(hù)生專(zhuān)業(yè)價(jià)值觀初始問(wèn)卷:在文獻(xiàn)研究的基礎(chǔ)上,探討《大醫(yī)精誠(chéng)》中的醫(yī)學(xué)倫理思想和護(hù)理倫理學(xué)相關(guān)理論,初步擬定護(hù)理專(zhuān)業(yè)價(jià)值觀量表的內(nèi)部結(jié)構(gòu),通過(guò)深入訪談的方法編制量表?xiàng)l目,最終形成由4個(gè)維度、56個(gè)條目組成的初始問(wèn)卷。 護(hù)生專(zhuān)業(yè)價(jià)值觀正式量表:使用初始問(wèn)卷對(duì)360名護(hù)生進(jìn)行預(yù)測(cè),通過(guò)統(tǒng)計(jì)分析,最終形成4個(gè)維度、27個(gè)條目的護(hù)生專(zhuān)業(yè)價(jià)值觀正式量表。量表包括專(zhuān)業(yè)自我成長(zhǎng)、利他主義、職業(yè)主動(dòng)性和專(zhuān)業(yè)使命感四個(gè)維度,四個(gè)維度的累計(jì)貢獻(xiàn)率為54.270%。護(hù)生專(zhuān)業(yè)價(jià)值觀量表和各維度的Cronbach's a系數(shù)為0.745-0.939,重測(cè)信度系數(shù)為0.862(P0.01),證明此量表信效度良好。量表采用5級(jí)評(píng)分,分值越高表明被試對(duì)象的專(zhuān)業(yè)價(jià)值觀水平越高。 護(hù)生專(zhuān)業(yè)價(jià)值觀總體現(xiàn)狀:結(jié)果顯示被調(diào)查的本科護(hù)生專(zhuān)業(yè)價(jià)值觀認(rèn)同度較高。量表各個(gè)條目的平均分值為3.09±0.87—4.03±0.79,護(hù)生專(zhuān)業(yè)價(jià)值觀各個(gè)維度的均值為3.56±0.64—3.97±0.59,四個(gè)維度中“利他主義”的均值最高,“專(zhuān)業(yè)使命感”的均值最低。 護(hù)生專(zhuān)業(yè)價(jià)值觀的影響因素:以護(hù)生專(zhuān)業(yè)價(jià)值觀量表調(diào)查得分為因變量,將調(diào)查結(jié)果中護(hù)生的一般資料(年級(jí)、家庭背景、政治面貌、是否獨(dú)生子女)作為預(yù)測(cè)變量,采用逐步多元回歸分析法進(jìn)行數(shù)據(jù)分析,結(jié)果僅有年級(jí)和家庭背景進(jìn)入回歸方程,但是由于決定系數(shù)R2僅為2.3%和3.9%,由于對(duì)因變量解釋率較低,所以不能確定兩者就是護(hù)生專(zhuān)業(yè)價(jià)值觀的影響因素。 [結(jié)論] 本研究編制的護(hù)生專(zhuān)業(yè)價(jià)值觀量表具有良好的信效度,其內(nèi)容符合我國(guó)文化背景和基本國(guó)情,可以用于測(cè)量我國(guó)護(hù)理專(zhuān)業(yè)本科學(xué)生的專(zhuān)業(yè)價(jià)值觀。調(diào)查結(jié)果顯示被調(diào)查的三所高校本科護(hù)生的專(zhuān)業(yè)價(jià)值觀水平較高!洞筢t(yī)精誠(chéng)》中所隱含的醫(yī)學(xué)倫理思想與我國(guó)護(hù)理專(zhuān)業(yè)價(jià)值觀內(nèi)涵聯(lián)系密切,可以在今后的護(hù)理教育中有意識(shí)地將其滲透到護(hù)生的人文教育中來(lái),不斷培養(yǎng)適合我國(guó)文化背景和基本國(guó)情且具有中國(guó)特色的優(yōu)秀護(hù)理人才。
[Abstract]:[Objective]
In this study, "medical ethics embodied in the" big medicine sincere connotation to enrich the basic framework of nursing ethics, and on the basis of the construction of nursing in accordance with China's cultural background and the basic situation of the value structure of the scale, and then compiled in accordance with the characteristics of Chinese culture of nursing students professional values scale for nursing education in China in the future to carry out the study on improving the professional values of nursing students and provide appropriate evaluation tools. On the other hand, developed in the research of nursing students professional values scale as the measurement tool for investigation, the notion of the professional value of undergraduate nursing students at school to understand, and to explore the influencing factors may exist.
[method]
By reviewing the related literature at home and abroad of nursing professional values in this study, to explore the theory of medical ethics and "nursing ethics related big medicine sincere > in the theoretical framework of this research. Then in" big medicine sincere > in the connotation of medical ethics as the basis for preparing interviews conducted in-depth interviews of nursing students and the nurses, so as to obtain the item scale and the formation of professional values of nursing students at the end of the initial questionnaire. Exploratory factor analysis was used to analyze the data, get professional values of nursing students of the scale, reliability and validity were tested on it.
In this study the nursing students professional values scale as a tool for Beijing, 894 nursing undergraduates in three Tianjin and other advanced medical universities were investigated. The statistics of the data obtained by SPSS statistical software, to further verify the new structure of the fitting degree of the scale, and the factors that influence the basic situation of professional values of nursing students and there may be analyzed.

Professional values of nursing students: the initial questionnaire based on literature research, to explore the theory of medical ethics and "nursing ethics related big medicine sincere >, initial development of nursing professional values within the structure of the scale, through the preparation methods of in-depth interview scale items, finally formed by 4 dimensions, the initial questionnaire was 56 items.
Professional values of nursing students: a formal questionnaire using the initial questionnaire to 360 nursing students, through statistical analysis, the final form of 4 dimensions, 27 items of nursing students professional values of the formal scale. The scale includes professional self-development, altruism, initiative and professional occupation mission four dimensions. The cumulative contribution rate of 54.270%. of the four dimensions of nursing students professional values scale and each dimension of Cronbach's a coefficient is 0.745-0.939, the test-retest reliability coefficient was 0.862 (P0.01), proved that the scale had good reliability and validity scale. By 5 score, the higher the score showed that subjects professional level is the value of the object view high.
Professional values of nursing students overall situation: the survey shows that the professional value of undergraduate nursing students a high degree of recognition. The average score of each item is 3.09 + 0.87, 4.03 + 0.79, the average professional values of nursing students in each dimension is 3.56 + 0.64 and 3.97 + 0.59, four dimensions of "altruism" the highest average, the lowest average professional mission ".
Factors affecting the professional values of nursing students in nursing students professional values questionnaire score as the dependent variable, the result of investigation of nursing students in the general information (grade, family background, political affiliation, singleton) as predictor variables, using multiple regression analysis method for data analysis, and the results only grade family background into the regression equation, but due to the coefficient of determination R2 was only 2.3% and 3.9%, as the dependent variables explain the low rate, so we can not determine the influence factors of the two is the professional values of nursing students.
[Conclusion]
The establishment of the professional values of nursing students scale has good reliability and validity, its contents accord with China's cultural background and basic conditions, can be used for measuring the professional value of undergraduate nursing students in China. The survey results show that the medical ethics surveyed three universities of undergraduate nursing students professional values of higher level implied. < > in the doctors and nursing professional values of our country closely, can be in nursing education in the future will consciously penetrate into the humanistic education of nursing students to develop suitable for China's cultural background and basic conditions and excellent nursing talents with China characteristics.
