優(yōu)化農(nóng)村基層醫(yī)療衛(wèi)生人力資源配置的對策研究

【摘要】:中國城市和鄉(xiāng)村之間的差距一直很大,在我國的醫(yī)療服務(wù)市場上,城市醫(yī)院集中了大部分的醫(yī)療資源,尤其是人力資源,服務(wù)能力平均而言相對較高,而農(nóng)村卻總是面臨缺醫(yī)少藥的困境。農(nóng)村的基層醫(yī)療衛(wèi)生人員不僅在數(shù)量上不夠,還總是面臨素質(zhì)低、技術(shù)差等各種問題,因而農(nóng)村醫(yī)療衛(wèi)生工作的改革始終是醫(yī)改的重點(diǎn)。本文運(yùn)用人力資源配置理論和公共產(chǎn)品理論對我國農(nóng)村基層醫(yī)療衛(wèi)生人力資源配置的現(xiàn)狀進(jìn)行了解讀,針對我國城市和鄉(xiāng)村醫(yī)療衛(wèi)生人力資源、大醫(yī)院與基層醫(yī)療機(jī)構(gòu)醫(yī)療衛(wèi)生人力資源配置不均衡的問題進(jìn)行了探討,通過對我國2010-2014年衛(wèi)生計(jì)生年鑒中醫(yī)療衛(wèi)生人才的統(tǒng)計(jì)數(shù)據(jù)進(jìn)行對比并以紅島經(jīng)濟(jì)區(qū)河套街道、紅島街道農(nóng)村衛(wèi)生人力資源情況為實(shí)例進(jìn)行分析,指出我國農(nóng)村基層醫(yī)療衛(wèi)生人力資源存在素質(zhì)有待提高、結(jié)構(gòu)配置不合理、人才流動(dòng)性大、隊(duì)伍不穩(wěn)定等問題,對問題進(jìn)行了剖析并提出了相應(yīng)的對策建議。本文認(rèn)為政府應(yīng)加強(qiáng)主導(dǎo)作用,優(yōu)化軟硬件的政府投入、合理規(guī)劃引導(dǎo)、加強(qiáng)機(jī)制建設(shè)等以促進(jìn)醫(yī)療衛(wèi)生人力資源向農(nóng)村基層流動(dòng);國家應(yīng)從醫(yī)療衛(wèi)生人才的培養(yǎng)和引進(jìn)、醫(yī)療衛(wèi)生人事制度改革、加強(qiáng)醫(yī)學(xué)院校源頭培養(yǎng)等方面加強(qiáng)基層醫(yī)療衛(wèi)生人才的引進(jìn)和培育;還從城市和鄉(xiāng)村醫(yī)師對口支援、推進(jìn)區(qū)域醫(yī)療聯(lián)合體、推進(jìn)醫(yī)療衛(wèi)生信息化建設(shè)等方面探討優(yōu)化農(nóng)村基層醫(yī)療人力資源配置的對策。
[Abstract]:The gap between cities and villages in China has been very large. In China's medical service market, urban hospitals concentrate most of their medical resources, especially human resources, and their service capacity is relatively high on average. However, the rural areas are always faced with the dilemma of lack of medical care and medicine. The rural primary medical and health personnel are not only insufficient in number, but also faced with various problems, such as low quality, poor technology, and so on. Therefore, the reform of rural medical and health work has always been the focus of medical reform. This paper uses the theory of human resource allocation and the theory of public products to interpret the present situation of rural primary medical and health human resources allocation in our country. This paper probes into the imbalance of medical and health human resources allocation between large hospitals and primary medical institutions in China's urban and rural medical and health human resources. By comparing the statistical data of medical and health talents in the 2010-2014 yearbook of health and family planning in China, and taking the rural health human resources of Hetao Street and Red Island Street in the Red Island Economic Zone as an example, It is pointed out that the quality of rural primary medical and health human resources needs to be improved, the structure is unreasonable, the talent mobility is large, and the ranks are unstable. This paper analyzes the problems and puts forward the corresponding countermeasures and suggestions. This paper holds that the government should strengthen the leading role, optimize the government input of software and hardware, and reasonably plan and guide. Strengthening the construction of mechanism to promote the flow of medical and health human resources to the rural grassroots, the state should start with the training and introduction of medical and health personnel, the reform of the medical and health personnel system, To strengthen the introduction and cultivation of primary medical and health personnel in areas such as the source training of medical colleges and universities, and to promote regional medical consortia through the counterpart support of doctors from urban and rural areas, This paper probes into the countermeasures of optimizing the allocation of medical human resources in rural areas from the aspects of promoting the construction of medical and health informatization.
