【摘要】：目的:觀察徐氏參芪苡術(shù)湯聯(lián)合化療對(duì)胃癌術(shù)后的臨床癥狀、生活質(zhì)量、化療所致毒副反應(yīng)等方面的影響,客觀評(píng)價(jià)其療效。方法:收集60例Ⅱ-Ⅲ期胃癌根治術(shù)后的患者,隨機(jī)分成兩組,各30例。對(duì)照組單純給予化療(XELOX或SOX方案),每個(gè)周期21天,觀察4個(gè)周期;治療組在化療基礎(chǔ)上服用徐氏參芪苡術(shù)湯,并隨證加減,水煎服,每日早晚飯后一個(gè)半小時(shí)服用。化療前2天和第四周期治療結(jié)束后的1周內(nèi)分別記錄患者的臨床癥狀、生活質(zhì)量、體重、血常規(guī)、肝腎功能、消化道及神經(jīng)系統(tǒng)副反應(yīng)等情況。用SPSS19.0數(shù)據(jù)軟件統(tǒng)計(jì)分析其結(jié)果,比較兩組治療前后的變化。結(jié)果:1.治療后臨床癥狀總積分治療組低于對(duì)照組(P0.05),治療組治療后低于治療前(P0.05),對(duì)照組治療前后比較無(wú)統(tǒng)計(jì)學(xué)差異(P0.05)。治療組胃脘痞脹、食少、便溏、乏力積分治療后均低于治療前(P0.05);對(duì)照組泛酸、便溏積分治療后均高于治療前(P0.05),便結(jié)積分治療后低于治療前(P0.05);治療后,胃痛、胃脘痞脹、食少、泛酸、便溏、乏力積分治療組均低于對(duì)照組(P0.05)。2.治療后生活質(zhì)量評(píng)分積分治療組低于對(duì)照組(P0.05),治療組治療后低于治療前(P0.05)。3.治療后KPS評(píng)分治療組高于對(duì)照組(P0.05),治療組治療后高于治療前(P0.05),并且治療組KPS評(píng)分療效評(píng)價(jià)優(yōu)于對(duì)照組(P0.05)。4.兩組患者體重治療前后、治療組治療前后變化均無(wú)統(tǒng)計(jì)學(xué)差異(P0.05),對(duì)照組治療后低于治療前(P0.05)。5.兩組患者治療后發(fā)生血液系統(tǒng)、腎功能、神經(jīng)系統(tǒng)毒性例數(shù)比較無(wú)統(tǒng)計(jì)學(xué)差異(P0.05);但是發(fā)生惡心嘔吐、腹瀉例數(shù)治療組少于對(duì)照組(P0.05)。結(jié)論:徐氏參芪苡術(shù)湯聯(lián)合化療可以改善患者胃痛、胃脘痞脹、食少、泛酸、便溏、乏力等方面的癥狀,改善機(jī)體功能狀況,減輕化療所致的惡心嘔吐、腹瀉等不良反應(yīng),提高胃癌術(shù)后患者的生活質(zhì)量。
[Abstract]:Objective: to observe the effect of Xu's Shenqi Yizhu decoction combined with chemotherapy on the clinical symptoms, quality of life and side effects caused by chemotherapy in gastric cancer after operation, and to evaluate the curative effect objectively. Methods: sixty patients with stage II-III gastric cancer after radical resection were randomly divided into two groups with 30 cases each. The control group was given chemotherapy alone (XELOX or SOX regimen) for 21 days each cycle and observed 4 cycles, while the treatment group was treated with Xu's Shenqi Yizhu decoction on the basis of chemotherapy, which was taken one and a half hours after meals in the morning and evening. The clinical symptoms, quality of life, body weight, blood routine, liver and kidney function, digestive tract and nervous system side effects were recorded 2 days before chemotherapy and 1 week after the end of the fourth cycle of treatment. The results were statistically analyzed by SPSS19.0 software, and the changes of the two groups before and after treatment were compared. Result: 1. After treatment, the total score of clinical symptoms in the treatment group was lower than that in the control group (P 0.05), and that in the treatment group was lower than that before treatment (P 0.05). There was no significant difference in the control group before and after treatment (P 0.05). The treatment group epigastric distension, less food, loose stools, fatigue score after treatment were lower than before treatment (P 0.05); control group pan-acid, loose stool score after treatment were higher than before treatment (P 0.05), stool knot score after treatment was lower than before treatment (P 0.05); after treatment, stomach pain, epigastric distension, less food, pan-acid, loose stool, fatigue score were all lower than the control group (P 0.05). After treatment, the score of quality of life in the treatment group was lower than that in the control group (P 0.05), and that in the treatment group was lower than that before treatment (P 0.05). After treatment, the KPS score in the treatment group was higher than that in the control group (P 0.05), and that in the treatment group was higher than that before treatment (P 0.05), and the curative effect evaluation of KPS score in the treatment group was better than that in the control group (P 0.05). 4. There was no significant difference in body weight between the two groups before and after treatment (P 0.05), but it in the control group was lower than that before treatment (P 0.05). There was no significant difference in blood system, renal function and neurotoxicity between the two groups after treatment (P 0.05), but the number of nausea and vomiting and diarrhea in the treatment group was less than that in the control group (P 0.05). Conclusion: Xu's Shenqi Yizhu decoction combined with chemotherapy can improve the symptoms of stomachache, epigastric distension, less food, pan-acid, loose stool, fatigue and so on, improve the functional condition of the body, reduce the adverse reactions such as nausea, vomiting and diarrhea caused by chemotherapy, and improve the quality of life of patients with gastric cancer after operation.
