miR-221表達(dá)水平與肝癌分期、分型及預(yù)后的相關(guān)性研究

【摘要】:目的:原發(fā)性肝細(xì)胞癌(Hepatocellular carcinoma,HCC)是發(fā)病率較高的惡性腫瘤之一,尋找肝癌發(fā)生發(fā)展的相關(guān)基因,可為肝癌的診斷、治療提供理論基礎(chǔ)。micro RNA(mi RNA)是一類長度為18-22nt的內(nèi)源性非編碼小RNA,被證明與肝癌的發(fā)生息息相關(guān),其生物效應(yīng)是在轉(zhuǎn)錄階段抑制相關(guān)基因的表達(dá)。本研究應(yīng)用實(shí)時(shí)熒光定量PCR的方法檢測肝癌組織中mi R-221的表達(dá)水平,并統(tǒng)計(jì)分析mi R-221表達(dá)量與HCC患者的分期、分型及預(yù)后等臨床資料的關(guān)系。在此基礎(chǔ)上進(jìn)一步觀察阻斷mi R-221表達(dá)后的肝癌細(xì)胞的生物學(xué)特性的改變。方法:收集246例肝癌患者腫瘤及周圍非瘤組織手術(shù)標(biāo)本,運(yùn)用實(shí)時(shí)熒光定量PCR技術(shù)檢測mi R-221的表達(dá)水平。根據(jù)mi R-221在肝癌組織中的表達(dá)水平,將HCC患者分為mi R-221高表達(dá)組和低表達(dá)組,分析mi R-221表達(dá)量與HCC患者臨床資料之間的關(guān)系。設(shè)計(jì)mi R-221反義鏈,通過慢病毒載體轉(zhuǎn)染肝癌細(xì)胞株,以CCK-8、克隆形成、劃痕實(shí)驗(yàn)、Transwell、流式細(xì)胞周期檢測等觀察阻斷mi R-221表達(dá)后的肝癌細(xì)胞的生物學(xué)特性。Westernblot檢測阻斷Mi R-221對細(xì)胞周期調(diào)節(jié)蛋白p27~(kip1)、p57~(kip2)的影響。結(jié)果:mi R-221表達(dá)水平與腫瘤病理學(xué)分級(jí)(P=0.0001)、TNM分期(P=0.03)、門靜脈癌栓(P=0.001)、甲胎蛋白(P=0.007)、淋巴結(jié)轉(zhuǎn)移(P=0.001)密切相關(guān),但與年齡、性別、腫瘤大小、腫瘤數(shù)量、包膜形成等無關(guān),mi R-221高表達(dá)與HCC患者總體生存期負(fù)相關(guān),COX回歸生存分析結(jié)果顯示mi R-221是判斷HCC患者預(yù)后的一個(gè)獨(dú)立預(yù)測因子。阻斷mi R-221的表達(dá)并能夠有效地抑制肝癌細(xì)胞的增殖活動(dòng),也能夠阻止其克隆形成及遷移侵襲的能力。流式細(xì)胞周期檢測發(fā)現(xiàn)阻斷mi R-221的表達(dá)可以誘導(dǎo)肝癌細(xì)胞發(fā)生G1期阻滯。阻斷mi R-221的表達(dá)導(dǎo)致肝癌細(xì)胞中p57~(kip2)、p27~(kip1)蛋白表達(dá)量顯著增高。結(jié)論:mi R-221在HCC中表達(dá)水平升高,與HCC的發(fā)生發(fā)展具有密切關(guān)系,是HCC患者預(yù)后的一個(gè)負(fù)性獨(dú)立預(yù)測因子。阻斷肝癌細(xì)胞中mi R-221的表達(dá)將為肝癌的診斷、治療提供新的靶點(diǎn)。
