【摘要】：目的:分析多發(fā)性骨髓瘤患者校正血鈣的臨床意義。方法:檢測(cè)320例初發(fā)多發(fā)性骨髓瘤患者的血鈣水平,用同時(shí)采集靜脈血所得血清白蛋白濃度進(jìn)行校正,分析校正前后血鈣濃度的差異。結(jié)果:320例多發(fā)性骨髓瘤患者血鈣濃度為2.34±0.15 mmol/L,校正后血鈣濃度為2.6±0.17 mmol/L,高鈣血癥患者由11.3%升至校正后的23.1%,低鈣血癥患者由42.8%下降至校正后的7.8%,血鈣的正常患者例數(shù)增加。Ⅰ、Ⅱ和Ⅲ期病人的血鈣水平在經(jīng)白蛋白校正前后均有顯著性差異(P0.05)。320例MM患者貧血發(fā)生率為80%,腎功能不全發(fā)生率為20.9%,骨髓瘤骨病發(fā)生率為68.8%,貧血、腎功能不全、骨髓瘤骨病患者血鈣濃度均高于正常組,且校正后差異更顯著。接受化療的患者220例,中位PFS為15個(gè)月,中位OS為20個(gè)月,高鈣血癥患者PFS及OS縮短,校正后PFS及OS明顯縮短,具有顯著性差異(P0.001)。結(jié)論:校正血鈣能更敏感地反映疾病的嚴(yán)重程度,具有更好的預(yù)后判斷作用。
[Abstract]:Objective: to analyze the clinical significance of corrected serum calcium in patients with multiple myeloma. Methods: the serum calcium levels in 320 patients with primary multiple myeloma were measured. The serum albumin concentration was corrected by collecting venous blood simultaneously. The difference of serum calcium concentration before and after correction was analyzed. Results: the serum calcium concentration of 320 patients with multiple myeloma was 2.34 鹵0.15 mmol/L, corrected, and the serum calcium concentration was 2.6 鹵0.17 mmol/L, from 11.3% to 23.1%.
