幽門螺桿菌感染與胰腺癌關(guān)系的Meta分析

【摘要】:目的:探討幽門螺桿菌(Helicobacter pylori,H.pylori)感染與胰腺癌的關(guān)系。方法:檢索相關(guān)文獻(xiàn),對(duì)符合納入標(biāo)準(zhǔn)的文獻(xiàn)進(jìn)行資料提取,用Revman5.3軟件進(jìn)行meta分析,估計(jì)合并效應(yīng)量OR值及對(duì)應(yīng)的95%可信區(qū)間(95%CI),再繪制漏斗圖評(píng)價(jià)發(fā)表性偏倚。結(jié)果:共納入8篇文獻(xiàn),病例組和對(duì)照組共3133例。H.pylori感染與胰腺癌關(guān)系的合并OR值為1.27,95%CI:1.04-1.56,I2=24%。按照研究對(duì)象人種不同,分為東、西方兩個(gè)亞組,西方人群(OR=1.14,95%CI:0.91-1.42,I2=0%),表明H.pylori感染與胰腺癌沒有關(guān)系,但東方人群(OR=2.06,95%CI:1.29-3.29,I2=0%),表明H.pylori感染與胰腺癌有關(guān)系,且H.pylori感染是胰腺癌的危險(xiǎn)因素,倒漏斗圖示基本對(duì)稱。有6篇文獻(xiàn)檢測(cè)Cag A+,其合并OR值為1.32,95%CI:0.83-2.10,I2=72%,亞組分析示西方人群(OR=0.92,95%CI:0.73-1.16,I2=0%),表明Cag A+與胰腺癌沒有關(guān)系,但東方人群(OR=4.29,95%CI:2.14-8.60,I2=0%),表明Cag A+與胰腺癌有關(guān)系,且Cag A+可增加患胰腺癌的風(fēng)險(xiǎn)。結(jié)論:東方人群H.pylori感染和Cag A+可增加患胰腺癌的風(fēng)險(xiǎn),西方人群H.pylori感染和Cag A+與胰腺癌沒有關(guān)系。
[Abstract]:Objective: to investigate the relationship between Helicobacter pylori H.pylori infection and pancreatic cancer. Methods: the relevant literature was retrieved, and the data of the literature that met the inclusion criteria were extracted. The meta analysis was carried out with Revman5.3 software. The OR value of the combined effect and the 95% confidence interval (95% CI) were estimated, and then the funnel graph was drawn to evaluate the published bias. Results: a total of 8 articles were included. The OR value of 3133 cases of H.pylori infection and pancreatic cancer was 1.2795 CI: 1.04-1.56I224.4. According to the different race of the subjects, they were divided into two subgroups of east and west, and the Western population was 1.1495, and the H.pylori infection had no relationship with pancreatic cancer, but the oriental population had no relationship with pancreatic cancer, which indicated that H.pylori infection was associated with pancreatic cancer, and H.pylori infection was the risk factor of pancreatic cancer. The inverted funnel is basically symmetrical. Six papers examined Cag A, with a OR value of 1.32 鹵95CI0.83-2.10 and I2720.The subgroup analysis showed that Cag A had no relationship with pancreatic cancer, but Cag A had no relationship with pancreatic cancer, indicating that Cag A was associated with pancreatic cancer, and Cag A could increase the risk of pancreatic cancer. Conclusion: H.pylori infection and Cag A in oriental population can increase the risk of pancreatic cancer, but H.pylori infection and Cag A in western population are not associated with pancreatic cancer.
