【摘要】：目的:探討影響推廣應用子宮肌瘤高強度聚焦超聲(high intensity focused ultrasound,HIFU)消融治療的因素。方法:本研究數(shù)據(jù)資料來源于遂寧市中心醫(yī)院婦產(chǎn)科,通過對2015年2月至2015年9月前往四川省遂寧市中心醫(yī)院咨詢的1121名子宮肌瘤患者的相關(guān)信息和影響因素進行分析,指導患者現(xiàn)場填寫調(diào)查表格完成數(shù)據(jù)資料的收集,包括部分人口學相關(guān)信息、醫(yī)院診療流程相關(guān)信息和子宮肌瘤患者癥狀程度等。統(tǒng)計學上采用多因素logistic回歸分析和最大似然估計的向前逐步回歸法(forward LR)對影響患者治療選擇的因素進行分析。結(jié)果:1 121位患者中最終進行HIFU治療的患者399人,治療比例為35.59%。多因素分析顯示:醫(yī)師本人是否推薦進行HIFU治療(OR=4.519,95%CI=1.055~1.438,P=0.049)、醫(yī)師對HIFU的介紹是否規(guī)范(OR=2.202,95%CI=1.009~20.243,P=0.000)、患者是否了解子宮肌瘤的各項治療方式(OR=1.629,95%CI=1.539~3.153,P=0.006)、經(jīng)濟水平(OR=1.190,95%CI=1.044~1.356,P=0.009)、醫(yī)保種類(保險報銷幅度)(OR=1.356,95%CI=1.168~1.574,P=0.000)、癥狀程度(OR=1.232,95%CI=1.154~2.300,P=0.008)與患者是否選擇HIFU治療相關(guān)。結(jié)論:醫(yī)師本人是否推薦進行HIFU治療、醫(yī)師對HIFU的介紹是否規(guī)范、患者是否了解子宮肌瘤的各項治療方式、經(jīng)濟水平、醫(yī)保種類(保險報銷幅度)、癥狀程度是影響患者選擇HIFU治療的因素。參考以上因素的影響,有助于HIFU應用的推廣。
[Abstract]:Objective: to investigate the factors influencing the application of high intensity focused ultrasound (HIFU) in the treatment of uterine leiomyoma.Methods: the data of this study were collected from gynecology and obstetrics department of Suining central hospital. The information and influencing factors of 1121 patients with uterine leiomyoma who went to the central hospital of Suining city of Sichuan province from February 2015 to September 2015 were analyzed.Instruct the patients to complete the data collection by filling out the investigation form on the spot, including some demographics related information, the hospital diagnosis and treatment process related information and the hysteromyoma patient symptom degree and so on.Multivariate logistic regression analysis and forward stepwise regression with maximum likelihood estimation were used to analyze the factors affecting patients' treatment choices.Results of the 1 121 patients, 399 were treated with HIFU, and the proportion of treatment was 35.59.澶氬洜绱犲垎鏋愭樉紺，
