【摘要】：目的探討垂體后葉素-水囊分離法在腹腔鏡下卵巢囊腫剝除術(shù)中對(duì)卵巢功能的保護(hù)作用方法1.選取2014年12月~2015年12月就診于寧夏醫(yī)科大學(xué)總醫(yī)院婦科行腹腔鏡下單側(cè)卵巢囊腫剝除術(shù)的患者90例,將患者分為垂體后葉素組(術(shù)中局部注射稀釋的垂體后葉素)、生理鹽水組(術(shù)中局部注射生理鹽水)和空白對(duì)照組(傳統(tǒng)方法直接剝除囊腫)各30例。2.應(yīng)用ELISA法分別檢測(cè)垂體后葉素組、生理鹽水組及空白對(duì)照組患者術(shù)前、術(shù)后1月、3月、6月、12月血清AMH的表達(dá)。3.比較垂體后葉素組、生理鹽水組、空白對(duì)照組三組患者手術(shù)時(shí)間、手術(shù)前后血紅蛋白下降量及手術(shù)前后不同時(shí)間段血清AMH值的變化,使用SPSS 17.0版軟件包進(jìn)行相關(guān)統(tǒng)計(jì)分析。結(jié)果1.垂體后葉素組、生理鹽水組與空白對(duì)照組比較,三組手術(shù)時(shí)間分別為(47.03±11.94)、(48.07±13.02)、(53.87±15.82),差異無統(tǒng)計(jì)學(xué)意義(P0.05);三組手術(shù)前后血紅蛋白下降量分別為(0.59±0.29)、(0.49±0.24)、(0.65±0.31),差異無統(tǒng)計(jì)學(xué)意義(P0.05);2.術(shù)后1月、術(shù)后3月、術(shù)后6月三組患者血清AMH值均較術(shù)前血清AMH值降低,差異有統(tǒng)計(jì)學(xué)意義(P0.05);3.術(shù)后1月、3月垂體后葉素組、生理鹽水組AMH值大于空白對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義(P0.05),術(shù)后1月、3月垂體后葉素組血清AMH值與生理鹽水組比較,差異無統(tǒng)計(jì)學(xué)意義(P0.05)。4.術(shù)后6月三組患者血清AMH值不同,垂體后葉素組較空白對(duì)照組AMH值高,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。5.三組患者術(shù)后12月血清AMH值分別與術(shù)前比較,差異均無統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論腹腔鏡下卵巢囊腫剝除術(shù)后卵巢儲(chǔ)備功能會(huì)降低,使用垂體后葉素水囊分離有保護(hù)卵巢功能的作用,具有一定的臨床實(shí)用價(jià)值。
[Abstract]:Objective to investigate the protective effect of pituitrin-water capsule separation on ovarian function during laparoscopic excision of ovarian cysts.From December 2014 to December 2015, 90 patients underwent laparoscopic excision of ovarian cyst in gynecology department, General Hospital of Ningxia Medical University.The patients were divided into three groups: 30 cases in the control group, 30 cases in the control group and 30 cases in the control group, 30 cases in the control group and 30 cases in the group of intraoperative local injection of dilute pituitrin, 30 cases in the normal saline group and 30 cases in the control group respectively.The expression of serum AMH was detected by ELISA method in patients with pituitary hormone group, normal saline group and blank control group before, 1 month, 3 months, 6 months and 12 months after operation.The time of operation, the decrease of hemoglobin before and after operation and the changes of serum AMH in three groups were compared. The statistical analysis was carried out with SPSS 17.0 software package.Result 1.The serum AMH values of the three groups were significantly lower than that of the preoperative serum AMH in 1 month, 3 months and 6 months after operation, and the difference was statistically significant (P 0.05 / 3).The AMH values of the pituitary posterior leaf hormone group and the saline group were significantly higher than those of the blank control group at 1 month and 3 months after operation, and the difference was statistically significant (P 0.05). The serum AMH value of the pituitary posterior leaf hormone group was not significantly higher than that of the normal saline group at 1 month and 3 months after operation.At 6 months after operation, the serum AMH values of the three groups were different, and the AMH value of the pituitary posterior leaf hormone group was higher than that of the blank control group, and the difference was statistically significant (P 0.05).There was no significant difference in serum AMH between the three groups at 12 months after operation compared with that before operation (P 0.05).Conclusion the ovarian reserve function will be decreased after laparoscopic ovarian cystectomy, and the use of hypophyseal posterior leaf water sac can protect the ovarian function, which has certain clinical and practical value.
