【摘要】：目的:探討西黃膠囊輔助放化療治療中晚期食管癌患者的療效及不良反應(yīng),闡明其作用機制。方法:選擇未治的中晚期食管癌患者,通過篩選及剔除不滿足本研究要求者,最后入組患者114例,其中接受西黃膠囊聯(lián)合放化療同期治療的患者27例(聯(lián)合治療組);僅接受單純放化療治療患者87例,隨機抽取其中27例患者作為單純放化療組。觀察時間均為放療起始至第90天。比較2組患者生存質(zhì)量評分(KPS)、吞咽困難程度及急性放射性食管炎的發(fā)生率、發(fā)生時間和嚴(yán)重程度分級,分析患者腫瘤局控情況。結(jié)果:治療結(jié)束后,2組患者KPS評分均出現(xiàn)不同程度下降,但聯(lián)合治療組患者KPS評分明顯高于單純放化療組(P0.05)。治療中晚期階段,聯(lián)合治療組患者吞咽困難程度輕于單純放化療組(P0.05),急性放射性食管炎的發(fā)生率亦低于單純放化療組(P0.05),發(fā)生時間明顯遲于單純放化療組(P0.05),但2組患者急性放射性食管炎嚴(yán)重程度分級和腫瘤局控分級比較差異無統(tǒng)計學(xué)意義(P0.05)。結(jié)論:西黃膠囊對中晚期食管癌患者生存質(zhì)量起促進(jìn)作用,同時可緩解患者吞咽困難程度,降低急性放射性食管炎的發(fā)生率并延緩其發(fā)生時間,可作為中晚期食管癌患者放化療同期治療的良好輔助用藥。
[Abstract]:Objective: to investigate the efficacy and adverse reactions of Xihuang capsule in the treatment of middle and advanced esophageal carcinoma and to elucidate its mechanism.Methods: 114 patients with advanced esophageal cancer were selected by screening and culling the patients who did not meet the requirements of this study.Among them, 27 patients were treated with Xihuang capsule combined with radiotherapy and chemotherapy (combined treatment group), 87 patients received radiotherapy and chemotherapy alone, and 27 patients were randomly selected as radiotherapy and chemotherapy group.The observation time was from the beginning of radiotherapy to the 90th day.The quality of life score (QOL), dysphagia and the incidence, time and severity of acute radiation esophagitis were compared between the two groups.Results: the KPS scores of the two groups decreased in different degrees after the treatment, but the KPS score in the combined treatment group was significantly higher than that in the radiotherapy and chemotherapy group alone (P 0.05).In the middle and late stages of treatment,The degree of dysphagia in the combined treatment group was less than that in the radiotherapy and chemotherapy group, and the incidence of acute radiation esophagitis was also lower than that in the radiotherapy and chemotherapy alone group. The time of occurrence of acute radiation esophagitis in the combined treatment group was significantly later than that in the radiotherapy and chemotherapy alone group, but the acute radioactivity in both groups was lower than that in the chemotherapy alone group.There was no significant difference between the severity grade of tracheitis and the local control grade of tumor (P 0.05).Conclusion: Xihuang capsule can promote the quality of life of patients with advanced esophageal cancer, alleviate the degree of dysphagia, reduce the incidence of acute radiation esophagitis and delay the time of occurrence.It can be used as a good adjuvant drug for radiotherapy and chemotherapy in patients with advanced esophageal cancer.
