【摘要】：目的觀察分析腹腔鏡手術(shù)腸系膜上血管主干和屬支在十二指腸胰頭區(qū)域的解剖情況。方法選擇2016年7月至2017年1月北京協(xié)和醫(yī)院基本外科收治的共27例腹腔鏡右半結(jié)腸癌完整結(jié)腸系膜切除(CME)病人,觀察回結(jié)腸血管、右結(jié)腸血管、中結(jié)腸血管的動(dòng)靜脈出現(xiàn)率和毗鄰關(guān)系,以及胃結(jié)腸干的出現(xiàn)率和屬支情況。結(jié)果術(shù)中觀察發(fā)現(xiàn),回結(jié)腸血管出現(xiàn)率為100%,右結(jié)腸動(dòng)脈、靜脈的出現(xiàn)率分別為33.3%和11.1%,中結(jié)腸動(dòng)脈、靜脈的出現(xiàn)率分別為88.9%和92.6%。中結(jié)腸動(dòng)脈的長度為1.9(0~7.2)cm。胃結(jié)腸干的出現(xiàn)率為92.6%;由2~5支屬支匯合而成;主干的長度為0.8(0.2~2.4)cm。結(jié)論回結(jié)腸血管出現(xiàn)率高,可以做為手術(shù)解剖的首要步驟;腸系膜上血管在十二指腸胰頭區(qū)域的解剖結(jié)構(gòu)復(fù)雜,術(shù)者應(yīng)該充分了解該區(qū)域解剖關(guān)系。
[Abstract]:Objective To observe and analyze the laparoscopic surgery of superior mesenteric vascular anatomy in the trunk and branches of duodenum and pancreatic head area. Methods from July 2016 to January 2017 in Peking Union Medical College Hospital from a total of 27 cases of laparoscopic colorectal cancer complete mesocolic excision (CME) patients, observe the ileocolic vessels, right colic vessels, and adjacent moving rate the veins in the colon vessels, and the occurrence rate of gastrocolic trunk and branches. The results of intraoperative observation, ileocolic vessels occurred in 100%, right colic artery, vein occurrence rate were 33.3% and 11.1%, colonic artery, vein respectively for 88.9% and 92.6%. in colonic artery length 1.9 (0~7.2) cm. gastrocolic trunk the occurrence rate was 92.6%; by the 2~5 branch branches converging; trunk length is 0.8 (0.2~2.4) cm. conclusion ileocolon vascular rate is high, can be used as a surgical solution The first step of the caesarean section; the anatomy of the superior mesenteric vessels in the duodenum and the head of the pancreas is complex, and the surgeon should fully understand the anatomy of the region.
