【摘要】：目的目的探討腮腺腺泡細(xì)胞癌(ACC)的影像學(xué)特點(diǎn)及其與多形性腺瘤(PA)的CT及MRI鑒別診斷價值。方法回顧性分析17例腮腺ACC的CT及MRI表現(xiàn),對比分析其與61例PA的CT及MRI表現(xiàn)的差異,并行統(tǒng)計學(xué)檢驗(yàn)。結(jié)果與PA相比,ACC在年齡、包膜、微囊改變、周圍侵犯、ADC值(841.2±113.6/1285.1±244.6,P=0.000)及TIC曲線類型(A型:0/29;B型:1/0,P=0.000)方面差異均具有統(tǒng)計學(xué)意義(P0.05),而在位置、最大徑、邊界、形態(tài)、腫瘤密度/信號、強(qiáng)化、出血、鈣化、囊變及淋巴結(jié)腫大方面差異無統(tǒng)計學(xué)意義(P0.05)。C型TIC曲線及ADC值在二者鑒別的敏感性分別達(dá)75.0%、90.5%,特異性分別達(dá)96.7%、100.0%。結(jié)論 CT及MRI不僅有助于ACC與PA的鑒別診斷,而且在其術(shù)前分期及判斷預(yù)后中同樣具有較大價值。
[Abstract]:Objective to investigate the imaging features of parotid acinar cell carcinoma (ACCC) and the value of CT and MRI in differential diagnosis from pleomorphic adenoma (PAA). Methods CT and MRI findings of 17 cases with parotid ACC were retrospectively analyzed. The CT and MRI findings were compared with those in 61 cases of PA. Results compared with PA, the changes of age, capsule and microencapsulation were observed. There were significant differences in ADC value of 841.2 鹵113.6 / 1285.1 鹵244.6 / Pu (0.000) and TIC curve type A: 0 / 29B: 1 / 0 / 0 / 0.000), but there were significant differences in position, maximum diameter, boundary, shape, tumor density / signal, enhancement, hemorrhage, calcification. There was no significant difference in cystic degeneration and lymphadenopathy. The sensitivity of TIC curve and ADC value in the differential diagnosis between ACC and PA were 75.00.5,96.7g and 100.0.Conclusion CT and MRI are not only helpful in the differential diagnosis of ACC and PA, but also in the differential diagnosis of ACC and PA. It is also of great value in preoperative staging and prognosis.
