結(jié)直腸癌原發(fā)灶與肝轉(zhuǎn)移灶的擴(kuò)散成像比較

【摘要】:目的探討結(jié)直腸癌原發(fā)灶與其肝轉(zhuǎn)移灶之間在常規(guī)磁共振序列、擴(kuò)散加權(quán)成像(DWI)及表觀擴(kuò)散系數(shù)(ADC)上的不同。方法對經(jīng)病理或臨床證實(shí)的85例結(jié)直腸癌患者及23例結(jié)直腸癌肝轉(zhuǎn)移患者的磁共振表現(xiàn)進(jìn)行回顧性分析,并以病灶T1WI-r SI、T2WI-r SI、DWI-r SI平均信號強(qiáng)度、ADC值作為評價標(biāo)準(zhǔn)。統(tǒng)計學(xué)方法采用兩獨(dú)立樣本t檢驗(yàn)、Mann-Whitney秩和檢驗(yàn)及受試者工作特征(ROC)曲線圖。結(jié)果結(jié)直腸癌原發(fā)灶與肝轉(zhuǎn)移灶在T1WIr SI、T2WI-r SI、DWI-r SI平均信號強(qiáng)度及ADC之間具有顯著差異(Z=-7.333,P0.05;t=6.112,P0.05;t=-3.892,P0.05;t=7.518,P0.05)。在結(jié)直腸癌肝轉(zhuǎn)移灶的診斷中DWI-r SI與常規(guī)序列相對信號強(qiáng)度下的曲線面積(AUC)分別為0.940、0.786;在結(jié)直腸癌原發(fā)灶的診斷中DWI-r SI與常規(guī)序列相對信號強(qiáng)度的AUC分別為0.843、0.847。結(jié)論結(jié)直腸癌原發(fā)灶與肝轉(zhuǎn)移灶之間在不同MRI序列存在信號差異,說明兩者之間在其生物學(xué)特性上存在差異。DWI序列在對結(jié)直腸癌肝轉(zhuǎn)移灶的診斷準(zhǔn)確性高于常規(guī)序列,在原發(fā)灶的診斷準(zhǔn)確性上相近。
[Abstract]:Objective to study the conventional magnetic resonance imaging (MRI) sequence between primary colorectal cancer and its hepatic metastases. Diffusion-weighted imaging (DWI) and apparent diffusion coefficient (ADCC). Methods the MRI findings of 85 patients with colorectal cancer and 23 patients with hepatic metastasis were retrospectively analyzed. The mean signal intensity and ADC value of T1WI-r SI DWI-r SI DWI-r SI were used as the evaluation standard. The Mann-Whitney rank sum test and the ROC curve of subjects were used in two independent sample t tests. Results the primary focus and hepatic metastasis of colorectal cancer were studied. There were significant differences in the mean signal intensity and ADC between T1WIr SIT2WI-r SIN DWI-r SI and ADC. The curve area of DWI-r SI and conventional sequence was 0.9400.786in the diagnosis of liver metastases of colorectal cancer, and in the diagnosis of colorectal cancer, the mean signal intensity of DWI-r SI was 0.9400.786, and the mean signal intensity of DWI-r SI was 0.9400.786.The mean signal intensity of DWI-r SI was 0.9400.786in the diagnosis of liver metastases of colorectal cancer, and the mean signal intensity of DWI-r was 0.9400.786in the diagnosis of colorectal cancer. The relative signal intensities of DWI-r SI and conventional sequences were 0.843 鹵0.8477.Conclusion there are signal differences in different MRI sequences between primary and hepatic metastases of colorectal cancer. The results showed that the diagnostic accuracy of DWI sequence was higher than that of routine sequence in the diagnosis of liver metastases of colorectal cancer, and the diagnostic accuracy of primary tumor was similar.
